Covid-19 Procedures, Health, and Safety:

To protect the health of our staff we ask that you ensure you are in accordance with the following guidelines before booking an appointment. If you have an appointment reserved and do not meet these guidelines we ask that you reschedule your appointment to avoid a cancellation fee. Please do not book an appointment if you are not in accordance with the following;

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that Obsidian Hair Studio and its employees have put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, and all other communicable diseases/infections.

I further acknowledge that Obsidian Hair Studio and its employees can not guarantee that I will not become infected with the Coronavirus/Covid-19 or any other communicable disease/infection.

I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 or any other disease/infection may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.

I voluntarily seek services provided by my Hair Artist and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.

I attest that:
* I am not experiencing any symptoms of illness such as cough, shortness of breath, difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

For your comfort and safety, please notify your stylists if you have allergies, any physical issues or disabilities, or if you are pregnant. If you experience discomfort or any other symptoms during the treatment, please alert your service provider immediately.

Cancellation Policy:
I understand that things can come up. We value your time, and hope you value mine too. In the event you need to cancel your appointment, please be sure to notify me no later than 48 hours before your scheduled appointment. Failure to notify in time will incur a cancellation fee. To ensure our cancellation policy is upheld we require a credit card to book any appointment. Any appointment not cancelled within 48 hours of the scheduled date and time falls under the cancellation policy protocol. Any cancellations less than 48 hours are subject to a $25 per hour scheduled to be paid prior to rebooking any future appointments.

If you have any questions regarding this policy please communicate them before committing to an appointment.

Service Adjustment Policy:
Your satisfaction is the highest priority. If you are not completely content, please let me know during your service or within 7 days after your service has been performed. I am committed to making any needed corrections, as long as it falls within the services discussed and agreed upon during the consultation. We will be pleased to accommodate you free of charge in most circumstances. For said complimentary service to be done, this request must be made within 7 days of your initial appointment. We do not offer any refunds of any kind.

Product Return Policy:
A very lightly used product may be returned within 30 days from purchase. Make sure to reach out to your Hair Artist with your concerns as soon as possible after using the product.

Salon Etiquette:
Discriminatory/Derogatory statements and actions are prohibited in the salon and will not be tolerated. Our staff has the right to refuse service to anyone behaving improperly, intoxicated or if their state of health may influence the effects of the service.

Confidentiality and Privacy:
Every client has the right to privacy and confidentiality. The conversations you have with your stylist will remain confidential. Client information and email lists are confidential and will not be sold or shared with outside companies.

Booking Instructions:

Step 1: Once you're redirected to the booking site, you'll want to choose Your Hair Artist as the staff you'd like to book with.

Step 2: Find the section for your Hair Artist's services (Extensions, Cuts, or Color) in which you'll find the Booking Blocks to choose from.

Step 3: Select the Booking Block stated in the email in which you received this form.

Step 4: Follow the prompts to book your appointment.

If you're unable to book an appointment that works well for you, you will also be able to add yourself for potential dates for the Waitlist on Vagaro.

You'll also be receiving multiple follow up emails from me with more information on how to prepare for your appointment, so make sure to add erin@obsidianhairstudio.com to your contacts in your email and always check your junk mail to make sure you don't miss them!
