REHEARSAL SCHEDULE 

• Monday - Friday Camp begins May 31 and runs through August 1.
• Tech Week is July 20 - 27
• Performances are July 28 - July 31

List commitments that may interfere with rehearsals or performances. Please be specific about dates and times of conflicts.  

• NOTE: If you miss more than 2 rehearsals, You will be dismissed from Summer Camp and the show. (All absences must be listed below and MUST be approved in advance. If approved, the absence will be consideres excused. 
• ALL CAST & CREW MUST BE PRESENT DURING THE ENTIRE TECH WEEK.

Thank you for your interest in auditioning for our Free Summer Intensive Musical Theatre Summer Camp.

In order for your child to be a part of our Musical Theatre Intensive Summer Camp, your child must audition,
and successfully make it through the audition process.

A Day At Camp...  will last from 8:30am - 2:30pm and your child will participate the entire day.

WHAT TO PREPARE

ACTORS

One Short monologue, poem, or spoken word piece of your choice 30 seconds or longer 

SINGERS

One song of your choice - Uptempo or Slow (You don't have to sing the entire song) You may use a track from your phone, the track should NOT have anyone else singing. The only voice that we should hear singing is your child's. It is ok to sing a song from the show.

DANCERS

One - 1-minute dance piece, any dance style of your choice

If your child is multi-talented please make sure that your child is prepared to show off all of his or her talents! 

Callbacks will take place Monday, May 30th by invitation only. You will be informed via email if your child is invited to attend the callbacks.

Parent Committee membership - $50. (Parent will receieve 1 Parent T-Shirt for joining the committee.)

COVID 19 Waiver

I acknowledge the contagious nature of the Coronavirus/Covid-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Broadway in the Hood and A Source of Joy Theatricals, and affiliates have
put in place preventative measures to reduce the spread of the Coronavirus/Covid-19. I further acknowledge that Broadway in the Hood and A Source of Joy Theatricals, and affiliates cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/Covid-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, production staff, and other show participants and their families. I voluntarily am participating in Annie and acknowledge that I am increasing my risk to exposure to the Coronavirus/Covid-19. I acknowledge that I MUST comply with all set procedures to reduce the spread while participating in the workshop.

I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache,
sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States Of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/Covid-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non- contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the coronavirus/covid-19. Again, I understand the current health crisis in our country and agree to hold Broadway in the Hood and A Source of Joy Theatricals and affiliates harmless of any contracted virus and/or injury during participation of all activities such as attending rehearsals and performances of Annie. I understand the inherent risks of group settings and the possibility of getting sick. I waive my right to legally pursue Broadway in the Hood and A Source of Joy Theatricals, or any of its members and/or staff in any way, including the filing of any legal judgment or accusations. I have completely read, understood and agree to be bound by the above statement by signing this document I understand I am waiving my legal right to file a lawsuit by participating in activities, Broadway in the Hood and A Source of Joy Theatricals

Release for Use of Photos and Likeness by "Joy"

I understand that the A Source of Joy Theatricals, Inc (“JOY”) may take photographs, video, audiotape and other image and sound-based mediaof the company, including in public, and its staff, casts, and visitors. JOY may wish to use such photographs for educational, promotional,advertising, and other purposes. This permission for release, without compensation or prior notice, would allow JOY to use photographs in itsprinted publications, during presentations, and otherwise. Therefore, I hereby freely and voluntarily consent to the use and publication of myname, participation, picture, and/or likeness by JOY and/or its employees and/or agents for any and all purposes including, but not limited to,educational, promotional, advertising,and trade, through any medium or format, including, but not limited to, videotape, audiotape, film, photograph, television, radio, digital,internet, theater, or exhibition, at any time from this date forward until I revoke this consent in writing. I further waive any claims against JOYand/or its staff and/or agents based upon or related to its use or publication of my likeness, voice, participation, and/or picture. I freely give thisauthorization without expectation of compensation.

Activity Consent Form ( For Minor Only*)

This form is recommended for unit use to obtain approval and consent for A Source of Joy Theatricals,
Inc., Broadway in the Hood and guests (If Applicable) under 18 years of age to participate in production,
casting, culinary, stage hands, ushering, concessions, ticketing, hair and makeup, custodian, set-up and
tear down or any other duties concerning the production. It is recommended that parents keep a copy
of this form and contact the company manager in the event of any questions or in case emergency
contact is needed.

Hold Harmless Agreement

I UNDERSTAND THAT PARTICIPATION IN THE ACTIVITY INVOLVES A CERTAIN DEGREE OF RISK. I HAVE CAREFULLY CONSIDERED THE RISK INVOLVED AND HAVE GIVEN CONSENT FOR MYSELF OR MY CHILD TO PARTICIPATE IN THE ACTIVITY. I UNDERSTAND THAT PARTICIPATION IN THE ACTIVITY IS ENTIRELY
VOLUNTARY AND REQUIRES PARTICIPANTS TO ABIDE BY APPLICABLE RULES AND STANDARDS OF CONDUCT. I RELEASE A SOURCE OF JOY THEATRICALS, INC., BROADWAY IN THE HOOD, THE LOCAL COUNCIL, THE ACTIVITY COORDINATORS, AND ALL EMPLOYEES, VOLUNTEERS, RELATED PARTIES, OR OTHER ORGANIZATIONS ASSOCIATED WITH THE ACTIVITY FROM ANY AND ALL CLAIMS OR LIABILITY ARISING OUT OF THIS PARTICIPATION. IN CASE OF EMERGENCY INVOLVING MY CHILD, I UNDERSTAND EVERY EFFORT WILL BE MADE TO CONTACT ME. IN THE EVENT I CANNOT BE REACHED, I HEREBY GIVE MY PERMISSION TO THE MEDICAL PROVIDER SELECTED BY THE COMPANY MANAGER IN CHARGE TO SECURE PROPER TREATMENT,
INCLUDING HOSPITALIZATION, ANESTHESIA, SURGERY, OR INJECTIONS OF MEDICATION FOR MY CHILD. MEDICAL PROVIDERS ARE AUTHORIZED TO DISCLOSE TO THE COMPANY MANAGER EXAMINATION FINDINGS, TEST RESULTS, AND TREATMENT PROVIDED FOR PURPOSES OF MEDICAL EVALUATION OF THE PARTICIPANT, FOLLOW-UP AND COMMUNICATION WITH THE PARTICPANT’S PARENTS OR GUARDIAN, AND/OR DETERMINATION OF THE PARTICIPANT’S ABILITY TO CONTINUE IN THE PROGRAM ACTIVITIES.

In consideration of the sum of zero Dollar, ($0.00), receipt for which is hereby acknowledged, and for the opportunity to participate in the above-identified video program produced by [A Source of Joy Theatricals/Broadway In The Hood], I agree that the program(s) may be broadcast and distributed without limitation through any means and I shall not receive any compensation for my participation.
I confirm that any and all material furnished by me for this program is either my own or otherwise authorized for such use without obligation to me or any third party.  I also agree to the use of my name, likeness, portrait or pictures, voice and biographical material about me for educational, program or series publicity and organizational promotional purposes.
I further agree that my participation in the program confers upon me no rights to use, ownership or copyright.  I release [A Source Of Joy Theatricals/Broadway In The Hood], its employees, agents, and assigns from all liability which may arise from any and/or all claims by me or any third party in connection with my participation in the program(s).
It is understood that [A Source Of Joy Theatricals/Broadway In The Hood] is under no obligation to broadcast the above-identified program(s) or series.
