    My signature acknowledges that I have read and agree to receive the treatments or series of treatments listed above and that I will adhere to all of the aforementioned statements that I have confirmed. 

    For this and all future appointments at Yoali Studio I assume the responsibility to inform my service provider of any changes to the responses I have previously confirmed.

    Please read all of the following agreement carefully

    With my electronic signature I certify for this and all future appointments that all of the above information is true and accurate to the best of my knowledge. I take full responsibility for alerting my Esthetician to any physical or mental condition which would affect my service or results. I understand my treatment is therapeutic in nature and will alert my Esthetician to any discomfort.  

    I understand and acknowledge there are risks involved with the treatment of facials, peels, microdermabrasion, microcurrent, electrical skin treatments, and waxing.

    I have had the opportunity to ask questions regarding these risks and other possible complications.

    I understand any false or misleading information I have given may lead to undesired results and complications and hereby waive the Esthetician's liability if such results or complications occur.

    I further understand my failure to follow post care instructions may also lead to undesired results, complications, or effects and hereby waive the Esthetician's liability if such results or complications occur.

    In consideration for the service provider performing this and all procedures at Yoali Studio, I agree I will assume the risk and full responsibility for any and all injuries, losses, or damages, which might occur to me while I am undergoing this procedure or side effects I may experience after the procedure is performed.

    I understand that the service provider does not diagnose illness, disease, or any other physical or mental conditions.

    Any sexual misconduct exhibited by the Client will result in immediate termination of the session, and the client will be liable for payment of the scheduled appointment. To the maximum extent allowed by law,

    I agree to waive and release any and all present and future claims, suits or related causes of action against the Esthetician, service providers, owners, officers, employees, or agents for negligence, injury, loss, death, costs or other injuries or damages to me as a result of this procedure.

    I agree that this waiver and release form shall bind the members of my family and any spouse or domestic partner, if I am alive, as well as my estate, family, heirs, administrators, personal representatives or assigns if I am deceased, and shall be deemed as a "Release, Waiver, Discharge and Covenant" not to sue the Esthetician or any of the service providers of Yoali Studio & Yoali Beauty LLC. 

     

    I agree to the following cancellation, rescheduling, and no show policy 

    Yoali Studio does not accept same day cancellations, in the event of needing to reschedule outside of the 2 days prior to your appointment the rescheduling/cancellation/no show fee for your scheduled service will apply for each service booked, each service cancellation price varies, with the MINIMUM fee being $50.
    After your third cancellation/rescheduling Yoali Studio will discontinue offering you service.

    In the event of a "same day cancellation", which is considered a no show, to your scheduled service, the FULL AMOUNT of  each of the booked services will be charged. This includes any appointment canceled after close of business the day before your scheduled appointment


    All fees are non-refundable and non transferable.

    A card on file is required to schedule any and all services at Yoali Studio for all Service Providers of Yoali Beauty LLC


    Any client who no call no shows their scheduled appointment, after being charged, will be blocked from booking if the fee is not cleared that same day. Communication is required to be able to reschedule.

    If any client disputes charges resulted from Yoali Studio- cancellation, rescheduling, and no show policy, Client will be blocked from booking with all service providers at Yoali Studio indefinitely no exceptions.

     

    Yoali Studio Service provider and Yoali Beauty LLC reserves the right to refuse service at any time to non-compliant guests.

    I understand and hereby release harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that maybe caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from my Service Provider. I understand that this release discharges my Service Provider from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment , or property damage that may arise from, or in connection to, any services received from my Service Provider
    This liability waiver and release extends to the salon together with all owners, partners and employees.
