Volunteer Agreement and Waiver

Waiver and liability release for Memphis City Beautiful
By signing this waiver and release, I:

Release the City of Memphis Government, including but not limited to the Division of Community Enhancement, Memphis City Beautiful, and all agents/affiliates, from all claims, loss, damage (whether to person or to property), expenses, injuries, and/or illness while serving as a volunteer (which includes the use of tools and resources) for the aforementioned parties and programs.

Consent to the sponsor’s use of my name, likeness, or participation in the event for publicity purposes, including but not limited to print, broadcast, social media, etc., and acknowledge that I will be subscribed to Memphis City Beautiful’s email database with the option to unsubscribe at any time.

Will adhere to all recommended safety protocols as indicated at the beginning of the project.

Parents or legal guardians must sign this waiver for any volunteers under the 18 of age.
