CCSL Proxy for 2024 AGM

FORM OF PROXY FOR ANNUAL GENERAL MEETING OF MEMBERS

This proxy is solicited by the leadership of the Calgary Centre for Spiritual Living located at 4411 Manitoba Rd SE, Calgary, Alberta, a member of Centers for Spiritual Living, for the Annual General Meeting of the Membership to be held at 12:15 pm on Sunday the 21st day of April 2024.

I, the absentee member, hereby appoint

(please provide first and last name of the person you are appointing - who may be the Minister, a Board member, or any current member in good standing)

to act, and vote for and on behalf of myself and represent me as part of the quorum at the above-named AGM and at any adjournment thereof and on every ballot and hand vote that may take place in connection therewith. *Please note that both the absentee member and the nominee (the proxy voter) must be current (2024-2025) members of CCSL and you must instruct your nominee proxy how you wish to vote.

I understand that this form of proxy must be dated, signed and filed through this online form, prior to the commencement of the said meeting. (However, it saves a lot of last minute scrutineer work if you can complete, in advance before 12 noon on Thursday, April 11th and you are encouraged to do this). I further understand that this proxy will not be valid and will not be acted on or voted unless it is completed and received as outlined here.
