Costume Order Form Madeline 2022

All classes up through Level 3 will be ordered a costume (Combo classes will receive 2 costumes or one 2-in-1 costume.)

The girth measurement starts at one shoulder and goes through the crotch back to the same shoulder, creating a circle around the torso.This is the most important measurement.  You should allow for at least one inch of growth for the girth measurement of your child.

Every attempt will be made to order the costumes pictured on the costume chart, but sometimes certain costumes become unavailable. If that happens, the most suitable replacement will be
made.

Make sure you allow for growth.  It’s always easier to fix a costume that is too big than a costume that is too small. Any errors due to measurement are the responsibility of the parent.  If the company is able to make an exchange or alteration, the parent will be charged.
