The program of care or service demonstrates quality of life for the individuals served or significantly improves the aging services organization's operation, and/or
The program of care or service demonstrates innovation in workforce development, policies and practice, and
The program of care or service shows significant benefit to the quality of life of the residents or the quality of work life for the employees.
Eligibility: Any not-for-profit member of LeadingAge Indiana that provides care and services to the elderly.
