    Controlled Chaos Productions presents Earthdance at Maddox Ranch in Lakeland FL September 20th through September 23rd. We are seeking interactive booths, workshops, and vendors in the following areas: Art, Crafts, Dance, Wellness, Sustainability, and Food / Beverage.   • Guidelines

    1. Earthdance Lakeland is a rain or shine event. Booth fees and staff passes are non-refundable.
    2. All food vendors must present a Health Department Food License, a Florida Sales Tax License, and a Certificate of Liability Insurance for coverage no less than $300,000 the dates of Sept. 20th, 21st, 22nd, and 23rd. 2012 with Controlled Chaos Productions as the certificate holder/additional insured party.
    3. Booth space will not be sub-leased or combined with a business other than the company name / DBA named on this application. However, multiple Artists may collaborate under one booth name.
    4. All vehicles on the inside of the festival grounds must remain parked for the duration of the festival.
    5. Vendor booths must be manned by at least one person for the entire time the booth is open. Controlled Chaos Productions will be held harmless for theft or damage of equipment and/or inventory incurred during the festival.
    6. Vendors shall remove and properly dispose of all rubbish and booth materials by 6pm on Sunday the 23rd. Recycling and composting is encouraged. Food vendors’ grease must be removed from festival grounds by the vendor.
    7. Vendors must provide their own tables, displays, lights, and tents*. All items for sale must be visibly priced. Vendors may not sell any items with the Earthdance Logo. *Artists without tents may apply for room under Art Market Collective Tent
    8. Vendors may not sell or distribute alcoholic beverages. No styrofoam cups or disposable glass containers are allowed in the festival.
    9. No smoking is allowed in the food concession unit due to fire and health department regulations.
    10. All vendors rent booth space as independent contractors, not as employees or representatives of
    Earthdance Lakeland, and thus assume all responsibilities for withholding taxes, social security, and public
    liability, and signing waivers.
    11. Vendors using the festival’s electricity must provide extension cords for their electrical appliances. Alternatively, vendors may use their own generator.


    Schedule goes as follows:

    Thursday, September 20th ... SET UP DAY starts @ 3.pm. (vending optional)

    Friday, September 21st ... MARKET HOURS (12 noon - Midnight)

    Saturday, September 22nd. ...MARKET HOURS (12 noon - 2 a.m.)

    Sunday, September 23rd ...TEAR DOWN DAY  3pm-6pm (vending optional)

    By selecting "I agree" below, the vendor confirms having filled out a copy of the Vendor
    Application and having read the conditions of participation in Earthdance Lakeland. Payment options will be included in your Acceptance Letter
