  • CONFIDENTIALITY STATEMENT OF AGREEMENT

    As a Reach One Youth Organization volunteer whose duties include working with children, I understand that I may not disclose to any unauthorized person any confidential information that I may encounter while performing my duties, and that such breach of confidence may ressult in my release from the volunteer program of Reach One Youth Organization.

    • CHILD ABUSE & NEGLECT REPORTING REQUIREMENTS 
    • All child care personnel are mandated by law to report their suspicions of child abuse, neglect,  or abandonment to the Florida Abuse Hotline in accordance with s. 39.201 of the Florida  Statutes (F.S.). 

      * Child care personnel must be alert to the physical and behavioral indicators of child abuse and  neglect. “Child Abuse or Neglect” is defined in s. 39.201, F.S., as “harm or threatened harm” to a  child’s health (mental or physical) or welfare by the acts or omissions by a parent, adult household  member, other person responsible for the child’s welfare, or for purposes of reporting requirements  by any person. 

      Categories include: 

      - Physical Abuse or Neglect (i.e. unexplained bruises, hunger, lack of supervision...) - Emotional Abuse or Neglect (i.e. impairment in the ability to function, depression...) - Sexual Abuse (i.e. withdrawal, excessive crying, physical symptoms...) 

      * Reports must be made immediately to the Florida Abuse Hotline Information System by  - Telephone at 1-800-96-ABUSE (1-800-962-2873), or 

      - Fax at 1-800-914-0004, or  

      - Online at http://www.dcf.state.fl.us/abuse/report/. 

      * Failure to perform duties of a mandatory reporter pursuant to s. 39.201, F.S. constitutes a violation  of the standards in ss. 402.301-319, F.S. and is a felony of the third degree. Remember, it is each  child care personnel’s responsibility to report suspected abuse and/or neglect. 

      * All reports are confidential. However, persons who are mandated reporters (child care personnel)  are required to give their name when making a report. 

      * It is important to give as much identifying and factual information as possible when making a report. 

      * Any person, when acting in good faith, is immune from liability in accordance with s. 39.203(1)(a),  F.S. 

      * For more information about child abuse and neglect, visit the Department’s website at  www.myflorida.com/childcare and select “Training Requirements.” The Department offers a 4-hour  Identifying and Reporting Child Abuse and Neglect course for child care providers. This course is  an overview of the various types of abuse and neglect, indicators that may be observed, the legal  responsibility of mandatory reporters, and the proper procedure for reporting abuse and neglect, as  required by ss. 402.305(2) and 402.313(1), F.S. The course is offered both online and instructor based throughout Florida.

      • As a volunteer, you are an Ambassador for Reach One Sports and should treat athletes, guests, participants, and other volunteers with the utmost courtesy and respect.
      • Attire options include clean, khaki or athletic pants and comfortable shoes. We will provide volunteer shirts for your shifts (as they are available).
      • Be prepared to answer many types of questions. Familiarize yourself with the tasks and routines prior to your shift. You will get more instruction upon arrival.
      • Once you sign up, we depend on your commitment to being a part of Reach One Sports. Your prompt arrival and completion of your shift are vital to the success. When you sign up, you commit to being present for that specific shift(s) during the entire length of that shift(s).
      • If it is the day of your scheduled shift and you find you cannot make it, please be considerate and contact coach Billy or coach Sandra so we can make necessary adjustments.
      • Our events are subject to weather changes. Please check the weather and dress accordingly. We recommend having extra layer of clothing available as needed.
      • You may not consume any alcoholic beverages or smoke during your volunteer shift.
      • You may not text/talk on your cell phone during your volunteer shift. If you need to make a call, arrange to step away briefly.
      • Unless otherwise directed, you may not eat during your volunteer shift. Please plan on eating before/after your shift. 
      • Pets are not allowed with you during your volunteer shift.
      • Depending on game needs, volunteer assignments may be reallocated — please be flexible.
      • Reach One Sports reserves the right to search any bags for security reasons.
      • Reach One Sports is not responsible for any lost or stolen items.

      I understand I am making the following commitment based on the volunteer position of my choice: 

      During Fall and Spring Seasons:

      • Basketball Coaches – must be available 3 days/week for practices and games for the 6-week season
      • *Soccer Program Coordinator - 5-10 hours a week for 10 weeks (3 weeks pre-season, 6-week season, 1 week post season)
      • Soccer Coaches - must be available 3 days/week for practices and games for the 6-week season
      • Basketball and Soccer Referees – needed on gamedays (Saturdays) for 2-6 hours
      • Basketball and Soccer Scorekeepers - needed on gamedays (Saturdays) for 2-6 hours
      • Administrative Support – contact Sandra for specific hours/commitment
      • Media  (Help with sports photography, social media, etc.) – 3-5 hours/week, both remote and during games/practices
      • Fundraising/Concessions - needed on gamedays (Saturdays) for 2-6 hours
      • Reach Success Mentorship Program Facilitators – commit to a minimum of a 2 hour shift 1 time/week (more is available)  

      During Summer Seasons

      Summer program dates are following school closure and will be determined. However, hours are 7:30a-5:30p daily, with exceptions on holidays, campus closures and Friday hours.

      We would like to accept volunteers who work 40 hours a week as well as volunteers who can work 20 hours a week. We would like our volunteers to work at least two days a week. The work shifts will be 7:30 AM - 12:30 PM and 12:30 PM - 5:30 PM. 

      The age of our campers will range from 5 to 16 years old. We are located at ALARM International Church 2532 W Tharpe St. Tallahassee, Florida. 

      We need to fill the following summer volunteer positions:

      • Basketball, Soccer, and Flag Football Volunteer Coaches needed: Full-time (8-10 hrs/day) and part-time (5-6 hrs/day) shifts available over 9-weeks 
      • Administrative Support needed: Full-time (8-10 hrs/day) and part-time (5-6 hrs/day) shifts available over 9-weeks 
      • Media (Help with sports photography, social media, etc.): Part-time (5-6 hrs/day) shifts available over 9-weeks 

      I fully understand the guidelines and expectations of the volunteer position as described here, and will also adhere to my supervisor’s expectations where this job is concerned. I am able to perform all duties as outlined.
