קרן עשור - ג'ייגיב ע"ר, עמותה מס' 580586998, פועלת בישראל כארגון מוכר לצרכי זיכוי מס בהתאם לסעיף 46(א) לפקודת מס הכנסה. בנוסף, עמותת הידידים, JGive - Friends of Asor Fund USA, Inc, מספר זיהוי מס EIN: 81-0757923, פועלת בארצות הברית כארגון מוכר לצרכי זיכוי מס בהתאם לסעיף 501(c)(3) לקוד המס הפדרלי (להלן יחד: "JGive").

I, the undersigned, hereby declare to Keren Asor - JGive (Registration No. 580586998) that I have requested to refund the donation I made.

Asor Fund (R.A.), registration number 580586998, operates in Israel as a recognized organization for tax-deductible donations under Section 46(a) of the Income Tax Ordinance. Additionally, the Friends Association, JGive - Friends of Asor Fund USA, Inc, EIN: 81-0757923, operates in the United States as a recognized organization for tax-deductible donations under Section 501(c)(3) of the federal tax code (hereinafter together: "JGive").

עם החזרת הכספים שבוטלו, הקבלה תהיה בטלה.

אני מצהיר בזאת כי לא אדרוש בפני רשויות המס ו/או רשות אחרת לצורך הזדכות במס ו/או לא אעשה שום שימוש בקבלה אשר מספרה מצוין בטופס זה, שיצאה אגב תרומתי טרם ביטולה על ידי.

 

With the return of the canceled funds, the receipt will be void.

I hereby declare that I will not present a claim before the tax authorities and/or any other authority for a tax credit and/or make any use of the receipt specified in this form, which was issued in connection with my donation prior to its cancellation.
