1.CMC2 Transport & Rescue's adoption process.
2. I understand CMC2 Transport & Rescue will check my references: veterinary and personal.
3. I understand that each Puppy will have at least 2 Dhpp, rabies, bordetella, and been spayed/neutered. Each will have had flea and tick monthy prevention, dewormed 2 times, and be microchipped. Dog/Puppies 0ver 6 months old will have had a heartworm test done. Adults 1+year will have 1-Dhpp, rabies, bordetella,and be spayed/neutered,and be on monthy flea/tick prevention,and be dewormed at least 2 times. All pets will get heath certificates prior to transport.I understand my pet requre yearly rabies, distemper/parvo shots.
4. I understand that sometimes transport can be stressful on my pet and my pet will need to decompress before being introduced to the family, family pets, other pets, and going on outtings and My pet will need at least 3 months to really feel like they are at home and feel like they are apart of the family.
5. I understand that that sometimes with the stress of traveling to a new place that my pet develope a cough, have some anxiety, due to transports being stressful on them. I understand that Imay be asked to facetime with a representative from CMC2 transport and rescue, as home checks are not possible any other way
6. I agree to keep my pet on a leash if not in enclosed area- as to prevent loss
7. I understand that CMC2 transport and rescue will disclose all of the pets health and behavioral issues known to the rescue prior to adoption being completed. I must take my pet to my veterinarian clinc within the first 3 weeks for a health wellness check up and any other boosters needed.
8. I understand it is imperative to keep my pet on a monthy heartworm preventative and flea and tick preventative. It takes one mosquite bite from an infected mosquite for my dog to get the deadly,costly,and treatable sicken (Heartworms)
9. I understand my pet is to remain on monthly flea and tick prevention, it takes one tick to infect them with lyme, erlichea.. All pets come fully vetted (age appropriate)and adopter shall recieve a packet with all info included.
10. I understand that if i no longer want or can no longer care for my adopted pet, I agree to notify CMC2 Transport &Rescue, by phone or e-mail and provide a 30 day period to allow CMC2 Transport & Rescue to make arrargements to return the pet to rescue or an approved foster. I agree to keep the pet safe until transport can be arranged by rescue. Adopter will be responsible for getting proper paperwork to return to the states.
11. I agree to keep the pet as long as it shall live and will not transfer ownership of the pet except with consent from CMC2 Rescue.
Transport will arrive in Woodstock, New Brunswick
Waiver I agree to indemnify and hold harmless Cmc2 Transport & Rescue and all representative of said rescue ,against any losses, lawsuits, claims, injury, damages incurred by me or any persons of property by my adopted pet once the adoption is completed

IF THE DOG CANNOT REMAIN WITH YOU, YOU WILL IMMEDIATELY
RETURN IT TO A REPRESENTATIVE OF THE RESCUE. PLEASE CONTACT US IF YOU CANNOT KEEP THE DOG. IF YOU MOVE, NOTIFY US OF YOUR NEW ADDRESS AND PHONE NUMBER.
DUE TO THE NATURE OF AN OUT OF STATE ADOPTION, THE ADOPTERS HEREBY EXPRESSLY AGREE THAT SHOULD THEY DECIDE THAT THEY CAN NO LONGER MAINTAIN OWNERSHIP OF THE DOG, AND/OR THE DOG IS NOT WORKING OUT WITH ADOPTEES, THEY HEREBY AGREE THAT THEY WILL ALLOW THE DOG TO REMAIN IN THEIR HOME UNTIL TRANSPORT CAN BE ARRANGED FROM THEIR HOME BACK TO CMC2 RESCUE. IN LIEU OF THE DOG REMAINING IN THE ADOPTERS’ HOME, IF THE DOG CANNOT REMAIN IN THE HOME, THE ADOPTERS AGREE TO BOARD THE DOG AT THEIR EXPENSE AT A REPUTABLE VETERINARY CLINIC OR BOARDING FACILITY UNTIL TRANSPORT BACK TO CMC2 RESCUE IS ABLE TO BE ARRANGED WITH THE NEXT CMC2 RESCUE AND TRANSPORT. SHOULD THE DOG BE BOARDED, THE ADOPTERS ARE TO PROVIDE CMC2 RESCUE THE NAME AND CONTACT INFORMATION OF SAID FACILITY PRIOR TO THE DOG BEING RELOCATED THERE. I AGREE that should I void the contract in any way that my ADOPTION DONATION is NON-REFUNDABLE no matter the duration of the adoption. I agree and understand that the adopted dog can be taken from my care or home should this contract or agreement be breached in any way. This includes, not seeking a professional trainer when needed a behaviorist when the dog needs on any attempts at re-homing the animal, or failure to provide basic care to the adopted dog, abuse or neglect!
