Collecting your information: As part of our services we require to collect and maintain information provided by you in the course of your sessions. This information is kept securely and only accessed by your Occupational Therapist, and where necessary other authorised staff of the practice. This information is used to help inform treatment, and track progress and will not be used for any purposes other than to support your therapy.
Confidentiality: Maintaining confidentiality is a fundamental aspect of our practice and respected at all times. The information provided by you will remain confidential and may not be disclosed to any other parties without your informed consent. At times it may be helpful for your Occupational Therapist to communicat with your General Practitioner, Support Coordinatior, the NDIA, or other health care providers, however only relevant information will be shared, and only with your prior knowledge.
There are limits to confidentiality where law an Occupational Therapist is required to disclose information if:
Information is subpoenaed by a court
There is risk of harm to a child or minor
There is risk of harm to oneself or others
Clients attending who are minors will have their information provided to their parent or guardian as organised and discussed.
Services provided via telehealth are done so via a secure platform.
Appointments: Appointments will typically last between 50 - 60 minutes.
Fees:
Our fees are in line with the NDIA Price Guide which can be accessed here https://www.ndis.gov.au/providers/pricing-arrangements
Adaptive Counselling and Psychology Occupational Therapy hourly rate - $193.99
Travel time will be charged up to 30 minutes each way at the above rate.
All work related to service including but not limited to case notes, assessment/report write up, communication with external providers, and liaision with carers or care team, will be charged pro rata for the amount of time taken to perfom the service (e.g. 45 minutes of report writing = 145.49)
Invoices will be sent to the individual or plan manager and unless it is agreed otherwise payment is expected to be received within 7 days of receipt. Services may be suspended if payment arrears are accumulated.
Cancellation Policy:
We understand that it may not always be possible to provide ample notice of the need to cancel an appointment. If you are unable to attend your appointment and you do not give us at least 48 hours notice, in accordance with the NDIA price guide, the full fee of the session will be charged.
If you are under 18 years of age, your parent/s, or guardian will need be required to read this form provide consent.
