Liability Disclaimer & Notice
I individually & as a parent and or/ guardian of the Child Identified above hereby acknowledge the following notice and grant to Sarah Bane Yogitales the following:
Liability Release
Yogitales takes all reasonable care in ensuring that its programmes are safe.
However, I agree that my child will be engaging in physical activites that may involve some risk of inury. I acknowledge I have been advised to consult with my child's physician with respect to any past or present inury, illness, health problems or any other condition or medication that may affect my child's participation in the Yogitales programmes. I assume the above risks and accept responsibility for any injury sustained by my child and discharge and hold harmless Yogitales, its owners, staff & its suppliers from any liability arising from any injury to my child or other person or property caused by my child's participation in the Yogitales programs. I assume the above risks and accept responsibility if inury is caused either by mine or my child's own fault, or by a third party unconnected with Yogitales provision of services, or by events which Yogitales could not have foreseen or prevented even if they had taken all reasonable care.
Cancellation Policy
🐝To ensure a smooth experience for all families, a $20 holding deposit is required for each class in our 6-week program. This deposit will be deducted from your invoice after you attend the session.If your child is unwell, we’re happy to reschedule at short notice. ✅Otherwise, we kindly ask 24 hours notice for cancellations. If less notice is given, the $20 deposit will be held.Thank you for your understanding 🙏If you are happy to continue, an invoice will be sent out for your deposit, this is to be paid before your first session please.🐝
