Pre Employment Medical Exam at Hi-Precision Diagnostic Center
Charges will be covered by KMCIf you have a pre-existing medical condition or is currently under medication, kindly secure in advance a "Fit to Work Clearance" from your doctor.Fit to Work Clearances and/or additional or repeat laboratory tests are no longer covered by KMC, but you may complete this at the same preferred Hi-Precision clinic or any other clinic of your choice.If you have existing APE or PEME RESULTS (within 3 months), please submit them to your onboarding coordinator for PEME validation. Must include below results:Coverage of medical exam: basic laboratory test (Chest X-ray, Urinalysis, Complete Blood Count), Physical exam, and a mandatory drug test.No need to bring stool sample. Fasting is not required.Please bring at least 2 valid Government-issued IDs and a penFor rescheduling, you are only allowed to reschedule your appointment twice.Kindly avoid submitting multiple entries on this form.This is a first-come, first-serve basis. Please choose the earliest date possible to avoid delay in completion of exam.Monday appointment requests must be submitted on or before Friday at 4 PM. Endorsements or requests for immediate appointments are not permitted during the weekend.*
