NOTE: Providing consent does not initiate the survey, it merely allows you to schedule an interview time. Thank you for your interest in participating in this study. Please review the following information. The study is being conducted by a team of public health researchers from the Harvard T.H. Chan School of Public Health. We are trying to learn more about the impact of this year’s wildfires and get a better understanding of the needs of organizations responding to wildfires. We are particularly interested in hearing from you about your current situation. As part of the research, we would like to ask permission to interview you and record your responses. It is your choice if you want to participate. Participation is voluntary. You do not have to say yes, and even if you do, you can decide to stop at any time for any reason. Some of the questions might make you feel sad or scared, or bring back bad memories. You can take as much time as you need to answer. If there are any questions that make you uncomfortable or that you do not want to answer for any reason, you do not have to. If you want to stop at any time, the interview can be stopped.  Your responses will be recorded during the interview and analyzed by study staff, after which the recording will be destroyed. You will be asked to provide an email address so that we can contact you to arrange interview logistics. If you are uncomfortable using your primary email address, you can create an alternate email for use with this study. Your identity will be kept confidential, which means we won’t use it in the study, and no one will know which answers are yours in any analysis or publications unless you provide express permission.  You do not have to participate in this research, and you will not be compensated for your involvement. If you would like to participate, we would like to ask you questions in a private location to ensure your privacy and confidentiality is maintained; you will have the option of selecting an in-person interview in California, or a remote interview via secure video conference.
