Salon Thirty Seven PolicesCancellation Policy: I understand that things can come up. I value your time, and hope you value mine too. In the event you need to cancel your appointment, please be sure to notify me no later than 24 hours before your scheduled appointment. Failure to notify in time will incur a cancellation fee. To ensure our cancellation policy is upheld we require a credit card to book any appointment. Any appointment not cancelled within 24 hours of the scheduled date and time falls under my cancellation policy protocol. You will be charged 25% to your card on file of what the scheduled services were valued at. If you have any questions regarding this policy please communicate them before committing to an appointment. Saved Payment Method: All guests' payment methods are safely and securely stored on file using my POS system.Salon Thirty Seven LLC takes all cyber security measures to protect it’s guests privacy & security. Service Adjustment Policy: Your satisfaction is the highest priority. If you are not completely content, please let me know during your service or within 7 days after your service has been performed. I am committed to making any needed corrections, as long as it falls in the services discussed and agreed upon during the consultation. I will be pleased to accommodate you free of charge in most circumstances. For said complimentary service to be done, this request must be made within 7 days of your initial appointment. Product Return Policy: A very lightly used product may be returned by the next scheduled appointment you have with me for a credit toward that days appointment. Make sure to reach out to me with your concerns as soon as possible after using the product. Salon Etiquette: Discriminatory/Derogatory statements and actions are prohibited in the salon and will not be tolerated. Our staff has the right to refuse service to anyone behaving improperly, intoxicated or if their state of health may influence the effects of the service. Confidentiality and Privacy: Every client has the right to privacy and confidentiality. The conversations you have with your stylist will remain confidential. Client information and email lists are confidential and will not be sold or shared with outside companies. Salon Thirty Seven LLC takes all cyber security measures to ensure your information is secure.

Liabilities- please sign By reading and agreeing to this statement, I have read and understood all of Salon Thirty Seven LLC Policies found in the Digital Welcome Packet and stated in this memo. I agree to have my payment information saved and stored securely. I also understand that my chosen stylist of Salon Thirty Seven LLC and I will have an in depth conversation speaking about all aspects of today's appointment, in which I agree to and acknowledge, including price, and the expected physical and aesthetic results. I hereby release and agree to hold Salon Thirty Seven LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Salon Thirty Seven LLC. I understand that this release discharges Salon Thirty Seven LLC from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Salon Thirty Seven LLC. This liability waiver and release extends to the salon together with all owners, partners, and employees.

Model Release Waiver I understand that I am requesting model services from Salon Thirty Seven-LLC. I acknowledge that a Deposit is required to book any model service. As a model, I acknowledge that the artist selected to service me is in training. I understand that the outcome of my service may or may not be exactly what I desire and that the stylist is working to perfect a specific skill set. I acknowledge that all services will be performed at a 45% discounted price unless otherwise quoted. All service sales are final and there are no refunds or re-do’s for model services .
