Rapid Antigen Test (RAT) - Positive Result Declaration
You have tested positive for COVID 19 using a Rapid Antigen Test
Please use this form to report your positive Rapid Antigen Test (RAT) result.
Only complete this form if you are still in Tasmania.
You may complete this form for yourself or on behalf of another person
You do NOT need to fill in this form if you are waiting on a PCR test result or if it has been less than four weeks following your release from isolation if you previously tested positive to COVID-19.
You should complete a separate form for each person that tests positive in your household (including dependants e.g. children and people you care for).
Negative RAT results do not need to be reported.

You do not routinely need to have your RAT confirmed with a PCR test, unless you are:
more than 20 weeks pregnant
an Aboriginal and/or Torres Strait Islander aged 55 years and over
aged 16 years and over and unvaccinated or partially vaccinated (one dose only)
If you fit into one of these groups, you may be eligible for specific treatments that currently require a positive PCR test prior to treatment.
Please call the Public Health Hotline to organise your PCR test and advise them you may be eligible for specific treatment and need a PCR test.
