More Information:

Following tryouts, we will form teams of 10 players per roster. Season runs March 18th through July 31st (following Nationals). Practices will start March 6th, with each team having 2 practice days per week (Monday-Thursday) for a total of 3 hours a week.

Only two teams per age group will be created if numbers allow.

We will play in 8-10 tournaments total. Schedule will be released at Parent Meeting after tryouts.

Team Fee will be between $900-1200 depending on Game Schedule

Schedule will depend on how competitive the coaches think each individual team will be.

We will have competitive, intermediate and beginner schedule options.

Spring, fall and winter season package pricing will be available but not mandatory.

Payment Plans for all season will be available also.

Along with in season training packages.

CERTIFICATION OF FITNESS. I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in the A.C.E. Personal Training. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of the A.C.E. Personal Training in which I may participate and that it will govern my actions and responsibilities at Training/Camp. ASSUMPTION OF RISK, WAIVER, & INDEMNIFICATION In consideration of the privilege of participating in an organized event(s) with A.C.E. Personal Training, a North Carolina organization, and further recognizing the voluntary nature of my participation in this event, I, the undersigned person, intending to be legally bound, hereby promise to waive for myself, my guardians, heirs, executor, administrators, legal representatives and any other persons on my behalf, any and all rights and claims for damages, demands, and any other actions whatsoever, including those attributable to simple negligence, which I may have against any of the indemnitees identified below, which said injuries arise out of my participation in the activities comprising the aforesaid Trainings/Camps; as well as any use by me of any facilities accessed may be or offered to me for use by the organization. I FURTHER VERIFY THAT I HAVE FULL KNOWLEDGE OF THE NATURE AND EXTENT OF THE RISKS ASSOCIATED WITH ATTENDING. I EXPRESSLY, KNOWINGLY, AND VOLUNTARILY ASSUME THE RISKS INVOLVED IN THE PLANNED ACTIVITIES INCLUDING TRANSPORTATION TO AND FROM THE EVENT, AND AGREE TO HOLD THE INDEMNITEES HARMLESS FOR ANY RESULTING INJURY. I understand that, should I decline to execute this agreement, I will not be permitted to attend the organized event. I acknowledge that the Trainings/Camps may carry with it the potential for death, serious injury, and personal loss. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, coaches, and lack of hydration. I hereby assume all of the risks of participating in the Trainings/Camps, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of their possible liability without fault. I FURTHER AGREE AS FOLLOWS: (A)I WAIVE, RELEASE, AND DISCHARGE A.C.E. Personal Training, its directors, officers, managers, shareholders, employees, agent and/or assigns, Paul O’Neill Palmer III, including all other coaches, representatives, or volunteers (collectively “indemnitees”) from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from the A.C.E. Personal Training/Camps. (B) I INDEMNIFY, HOLD HARMLESS, PROMISE TO DEFEND, AND PROMISE NOT TO SUE the aforementioned indemnitees from any and all liabilities or claims, including costs of litigation and attorney’s fees, made as a result of participation in the A.C.E. Personal Trainings/Camps, whether caused by negligence or otherwise. I further agree to indemnify, hold harmless, and promise to defend the indemnitees from any all and liabilities or claims, including costs of litigation and attorney’s fees, arising out of my child/ward causing his or her own injuries, or if my child/ward causes injury to someone else. ADDITIONAL PROVISIONS. I consent and agree that A.C.E. Personal Trainings/Camps, and/or their coaches, agents, representatives or volunteers may take photographs or digital recordings of me as a participant during this event and use these in any and all media for training or promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims or interest and I understand that there will be no financial or other remuneration. This Agreement shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. This agreement shall be governed and construed by the laws of the State of North Carolina. Any action arising out of or in any way related to undersigned’s participation in or attendance at the Camp shall be filed in the Superior Court of Mecklenburg County, North Carolina. Undersigned hereby waives his right to any alternate jurisdiction or venue and grants exclusive jurisdiction of any disputes to said Court. If any provisions of this Agreement are deemed unenforceable, it shall not affect the enforceability of the remainder. I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL.*

Signature From Guardian PARENT/GUARDIAN ACKNOWLEDGEMENT FOR MINORS (under 18 years old) The Undersigned parent and or natural guardian does hereby represent that he/she is, in fact, acting in such capacity, is the legal parent/guardian of the participant, and has consented to his/her child or ward’s participation in the A.C.E. Personal Trainings/Camps, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect on lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.
