  • 변경하고자 하는 사항에 해당하는 사항을 기재, 서류 제출해주시기 바랍니다.

     

    ① 수련기관 기본정보(대표자,소재지 등) 변경 : 사업자등록증 첨부

    ② 수련생 정원 변경 : 영역별 수련 운영 계획서, 영역별 사회복지사업 수행 사실 확인서 첨부

    ③ 수련지도자 추가 : 수련지도자 확인서류(영역별 사회복지 실무경험 확인서 외 6개-하단의 제출서류 참고) 첨부

    ④ 기타 변경 : 협회 사무국(02-701-1877)으로 개별 문의
