  • Routine outings with the childminder permission

    I/we agree for the child named above to go on routine outings with the childminder named below. Routine outings will include:-

    Ø  Couple of mum and tots groups. (Tuesday, Wednesday, Thursday. )

    Ø  School run. (Monday- Friday)

    Ø  Trip to the park, shops , farm, garden centres and play areas, other childminders homes  ect. (any day)

    Observation permission

    I/we understand that ongoing observations will be undertaken of the child named above, to follow and assess their development, and will be displayed in their learning journeys. These may be in the form of written statements, photographs/videos or tape recordings.

    Outdoor play equipment permission

    I/we agree for the child named above to use play equipment in gardens, parks or playgrounds while in the care of the childminder named below.

    Transporting in a vehicle permission

    I/we agree for the child named below to be transported in a vehicle with the childminder named below.

    Photo permission

    I/we give permission for the childminder named below, or a person nominated by the childminder, to take photos (including those taken on a mobile phone or similar device) of the above-named child for the reasons I/we have indicated below:

    Ø  to record the child(ren)’s daily routine

    Ø  to record the child(ren)’s development

    Ø  to share with the child’s parents

    Ø  the childminder’s own album

    For any other reasons extra permission will be asked for.

    *Any photos stored electronically will be stored securely and deleted when the above named child is no longer in the childminder’s care.

    Photos of your child may appear in others learning journeys. Ie to show working together, friendships, sharing ect and group pictures.

    I/we give permission for the childminder named below to print photos of the child named above using a photo centre i.e. Asda photos/free prints or similar.

     

    Water play

    I/we give permission for the child named above to take part in water play under the supervision of the childminder named below, and provide a change of clothes for your child.

  • General Data Protection Regulations (GDPR)

    In order to comply with the Data protection legislation 2018 and GDPR all childcare providers are required to obtain permission for photos and give parents the option to withdraw that permission at any time. Parents must be kept informed of how, why and where their childrens photos are used, stored and shared. 

    In this setting, photos are only used for learning journeys. These photos are stored electronically on Tapestry, which is safe and sucure. I use my phone to take photos, whci are deleted immediately once they have been uploaded. 

    I share photos of childrens daily activites and develpment with parents using the daily diary on Whatsapp and Tapestry and will contunue to do so unless parents are no longer happy for me to do so.

    i am allowed to take photos of the children for their development under EYFS and for this purpose dont need parents permission unless there are muliple children in the photos and these are then put in other childrens learning journeys. I require permission from parents to both take and share group photos and this permission can be withdrawn at any time. once a childs learning journey leaves my setting i an no longer responisble for its destruction/deletion.

    permission must be given by parents if i wish to retin photos for personal use or memories unless these photos contain our own children in which case permissiion is not required. 

    photos- I understand photos will be taken within the setting for learnng and development purposes and these photos may contain multiple children. These photos will be shared through whatsapp and Tapestry.

    Medical- children emergancy medical details including photo will be carried with for medical/first aider to access. 

    Sharing info with other settings- we are required under the EYFS to share information with other settings and professionals about the children in our care and therefore need premisssio for this.

    facebook- I have a open facebook page where i often share photos of what the children have been doing, outings, crafts ect. Childrens faces will never appear on this page, but chldrens artwork, hands, feet ect may do with parents permission.
