RCOM Conversation Report Form

This Report Form is completed by any Member of the Regional Commission on the Order of Ministry (RCOM) to document a Conversation with an individual Candidate for Ordained Ministry. This includes a Candidate pursuing the Seminary Track to Ordination, the Apprentice Track to Ordination, Recognition of Standing & Ordination, UCC Partner Standing, or Reinstatement of Voluntary Lapsed Standing. This Report Form thoroughly documents the Conversation, including what was discussed as well as RCOM's affirmations and recommendations.

This completed Report Form will be stored in the Candidate's clergy file for future reference. Please contact your Ministry Associate in the Springfield Regional Office at sgf@ccmadisciples.org should you have any questions. Thank you so much!
