Appendix 17 is completed by any Member of the Regional Commission on the Order of Ministry (RCOM) to document a Conversation with a Candidate for Ordained Ministry. This includes all Candidates pursuing the Seminary Track to Ordination, Apprentice Track to Ordination, Commissioned Minister Track to Ordination, Recognition of Standing & Ordination, UCC Partner Standing, and/or Reinstatement of Voluntary Lapsed Standing. This Report Form creates a thorough record of the discussion, RCOM's affirmations, and RCOM's recommendations.
