All participants must sign this document and a parent or legal guardian must also sign if the participant is under the age of 18. In consideration of being allowed to participate in any Merrimack River Watershed Council program, event or activity (including on- water,on-land, and other outdoor and indoor activities), I acknowledge and agree that the risk of injury is significant and I knowingly and freely assume all such risks both known and unknown and assume full responsibility for my participation. I willingly agree to comply with terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

While participating in any program or while on the premises of the Merrimack River Watershed Council or while a passenger in any vehicle or vessel used for purposes of participating in Merrimack River Watershed Council events: I for myself and heirs, executors, hold administrators (and on behalf of any other person whom I am a legal guardian or representative) hereby release, indemnify and harmless Merrimack River Watershed Council Inc., its employees, executives, board of directors, assigns, representatives, contractors, and lessors of premises used to conduct the event, from any and all claims, demands, losses, and liability arising out of or related to any Injury, disability, death or property damage.

Participants, therefore, must take responsibility for their own safety and for having the appropriate skills, physical conditioning, equipment and supplies for the activities involved in the event. I agree to wear a properly fastened personal flotation device (PFD) at all times while in or on the water.

I hereby grant Merrimack River Watershed Council permission to use, for promotional purposes only, any photograph or video taken of me (by them) during my participation in any Merrimack River Watershed Council activity.

PARTICIPANT AND PARENT OF A MINOR PARTICIPANT AGREE: HAVE CAREFULLY READ, UNDERSTAND AND VOLUNTARILY SIGN THIS DOCUMENT AND ACKNOWLEDGE THAT IT SHALL BE EFFECTIVE AND BINDING UPON ME, MY MINOR CHILDREN AND OTHER FAMILY MEMBERS, AND MY HEIRS, EXECUTORS, REPRESENTATIVES AND ESTATE. Parent(s) must sign below, both in their capacity as a participant, and as Parent(s) of any minor child identified below.
