Your business is very important to us.  Below is a questionnaire, please take your time and fill the form out. It is necessary that you be clear in what it is that you envision for your video. HOW THIS WORKS:
 
1.     We have serval types of videos, choose one.
2.     Each video is designed with or without graphics, based on your choice.
3.     You will be offered 5 stock pictures if needed. If you are not satisfied with the pictures selected, (with your consent) we must buy different ones to your liking the fee is $3.24 per picture.
4.     We ask that you provide your own written content. If we must provide your written content (or pitch) (upon consent) you will be charged an additional $100. 
5.     Make sure as you are laying out the written content on this form that you have it edited it before you give it to us. If you want us to edit it for you, it will cost $25.
6.      If you are getting more than one video fill out a separate form for each video.
7.     When you are ready you will receive an invoice reflecting the payment total. The down-payment is due upon receipt. Service will begin.
8.     After the video has been constructed based on the information received, you will be given 2 revisions to look it over and make additions, subtractions, and edits.
9.     After the 2nd revision you will receive the final invoice with updated calculations based on, i.e. pictures, written content, edits..
10.  Once the final invoice has been paid, and you still find errors or have additions or subtractions, you will be charged $25 each time someone has to go back into the video.
