Pura Luxe Smoothing Treatment Liability Waiver

I consent by signing below that I am receiving a smoothing treatment at The Lotus Room, and that I understand results are variable. Due to varying texture, type of hair, and density, the results are conditional. I have been thoroughly consulted on the three different options for the smoothing treatment, and understand my choices as so - Healing/Repair, Frizz Reduction, and finally, "Smooth/Straight." Timing is also based on the type of hair, and is also specific to the stylist performing it.

-- PURA LUXE INGREDIENTS & RESULTS --

By signing below, I understand that Pura Luxe Smoothing treatment is completely Formaldehyde and sulfate free. This is NOT A BRAZILIAN BLOWOUT OR TRADITIONAL KERATIN. "This is a breakthrough formula of amino-acids, which penetrate the hair, expands and weakens the sulfate bonds but does not break or damage the hair completely." (per puraluxe.com) Because of the more gentle process, it can not be GUARANTEED that it will deliver super smooth or straight results on some hair types. This is NOT a relaxer. This customizable formula can be done directly after a color or even before, however I have been made aware that adjustments may need to be made to my color as the process can lighten existing color, and this is not exclusive to the Pura Luxe process. Color may also process faster if done afterwards due to the cuticle being more open. Pura Luxe can be applied over relaxers, will not cause damage if overlapped, and can be repeated as often as one feels. The typical results may last up to 6 months, however it varies based on the hair type, aftercare, and customized formula chosen. Pure Luxe can be done over extensions, however it is not recommended to perform on top of taped or bonded extensions as this may loosen the adhesive, and by signing below I acknowledge this.

-- AFTER CARE OF PURA LUXE TREATMENT --

I acknowledge that I was instructed on how best to care for my hair after treatment, which includes but is not limited to: Using a sulfate free shampoo, as well as a weekly nourishing/deep treatment and heat protectant prior to styling, as well as a light serum after to replenish moisture to styled hair. This ensures maximum results longer. Going in the ocean or pool will not damage the treatment results, however excessive sun/water/heat may cause the results (which vary based on treatment chosen) to dissipate more quickly than hair that is under exposed to the elements. There is no wait to cleanse or get wet time afterwards, and going longer without doing so will not strengthen results.
