Appointment Request Form

If you are looking to receive a tattoo from me, please take a few minutes to fill out this application.

I currently charge $1100 per session. During a full sesson, please plan to be at the studio for 6 or more hours. Remember to account for a minimum of 3 weeks between sessions.

I currently require a $200 (NON REFUNDABLE) deposit to schedule.

Please keep in mind, I am only able to accomidate a limited number of projects each month.

Please be as descriptive about your tattoo ideas as you can.

Please acknowledge there is an overwhelming amount of inquiries. Thank you for your patience as we work through applications.

If your project is something I'm unable to take on at this moment, we like to recomend other local artists who specializes in what you're looking for. (Even if I'm not the one doing your tattoo, I still want you to be happy with it!
