Application requirements
1. A description of the Community Service (location and activities) in which you were involved during your junior and senior years.

2. A (350 word) essay- Dr. Martin Luther King Jr. was inspired by Mahatma Gandhi who stated, “Be the change you want to see in the world”. As you strive to achieve your career aspirations, how will your future endeavors impact social change?

3) Submit the names and email addresses for two references:
1. Community Service Leader
2. Faculty Member
Each reference will receive an email from United Ebony Society of Gwinnett County, Inc., to submit a letter of recommendation on your behalf.

Each letter of recommendation must include the following:
- The reference's name, occupation, phone number and email address
- The length of time the person submitting the reference has known and worked with you, along with a clear description of the relationship.
- A description of your character, leadership, and work ethic
- (For Community Service) The type of community service activities in which you have been involved, along with your specific role.
Please include the location(s) of your community service activities or volunteer work during your junior year and senior year in high school.*
