Medical and Behavioral Disclaimer:
Creekside Animal Rescue, Inc. hereinafer referred to be C.A.R., attempts to make sure every animal adopted is healthy but C.A.R. cannot guarantee the health of any animal. The adopter understands and agrees that the animal has been surgically sterilized (spayed or neutered). The adopter agrees that C.A.R. is not responsible for any medical expenses which may be incurred by the adopter once this agreement is signed. The animal is adopted "as is" and the adopter assumes all responsibility for treatment of any and all existing conditions or any other physical, behavioral, or tempermental changes which may occur. The adopter releases C.A.R. of any and all liability in regards to the animal they are adopting.
C.A.R. makes no explicit or implicit guarantees regarding temperment of the animal and/or whether the animal is housebroken. The adopter agrees that their new pet will be treated humanely at all times and will recieve proper care (food, shelter, attention, and medical care as needed). Failure to do so could result in the animal being reclaimed by C.A.R. Adopter also understands that C.A.R. will follow up via phone calls, texts, and/or emails to periodically ensure everything is going well.
It is further understood that the microchip registration will be done by C.A.R. once all paperwork has been recieved and we have processed the adoption. Adopter will recieve a series of emails from 24PetWatch stating that the chip registration is complete and the adopter is eligible to claim the FREE one month of pet insurance provided by C.A.R. Accepting the gift of one month FREE insurance is optional.
In the event the animal becomes lost and 24PetWatch is unable to contact the adopter, 24PetWatch will then contact C.A.R. It is further agreed that if C.A.R. is contacted by 24PetWatch, C.A.R. will make up to five attempts to contact the adopter via phone calls, texts, and/or emails regarding reclaiming the lost animal. If attempts to reach the adopter are unsuccessful, it will be assumed that the adopter no longer wants the animal and ownership of the animal will return to C.A.R.
