Course Information:

Come join in our exciting Kindergarten Program. New students must be evaluated prior to beginning class. We will be learning through games, singing, crafts, educational activities and making new friends! This program operates month by month, Monday - Friday. We will have 9 children per class. Drop off begins at 8:55am on the dot. We will have an early drop off available for an additional fee. Please send a NUT FREE snack and water bottle. We request no snacks that are in need of a utensil or items such as gogurt or capri suns. Also pack a full change of clothes. Registration and Payment Information:

Registration Forms are only accepted when accompanied by payment of the non-refundable supply fee in the amount of $150.00. If you submit a registration form without payment your child is not considered registered, and their spot will not be held. Please submit registration form digitally. Payment can be submitted via Venmo: Virginia Dill@virginia-dill or by check made out to VBSA and delivered to Samantha Marti in the main office. Monthly tuition is due each month in the amount of $510.00.
Additional Information Required:

Please send in a digital copy / digital screen shot of Birth Certificate issued by the state of child's birth, not a certificate of live birth from the hospital. We also require a digital copy / screenshot of current shot record and most recent physical.

I give my child permission to participate in the Christmas Program which will contain direct references to Jesus Christ. Signature*
I understand that if my child has a cough, fever, unexplained rash, any communicable disease, profuse runny nose from allergies or has vomited in the last 12 hours they are not allowed to attend school. Signature*
I understand that Virginia Beach School of the Arts is a NUT FREE school. I will not send peanuts or any other nut containing snack to school including granola bars. Signature*
I understand that only water is allowed in reusable bottles and sports cups at school. Signature*
I understand that yogurt, fruit cups, gogurts, and any snack that is wet, mushy or requires a spoon or poses a slipping hazard when spilled is not allowed. Only dry foods such as crackers, apples wedges, bananas, cheese or goldfish are permitted. Signature*
I understand and agree to remit the Preschool for the Arts / Homeschool Unlimited monthly tuition. Signature*
I understand that tuition in full is due monthly regardless of my child's attendance or lack thereof due to illness, vacations, holidays or inclement weather. Signature*
I give Virginia Beach School of the Arts staff permission to take my child on nature walks to the Francis Land House garden, property and park. I understand that this property is adjacent and involves crossing King's Grand Road. I realize that these places are public property. Signature*
I give my child permission to play on the property of Virginia Beach School of the Arts which includes a tire swing, tree climbing and other outdoor activities. I will not hold the Virginia Beach School of the Arts and its staff liable in case of injury while playing outside. Signature*
I understand that if my child withdraws, the full monthly amount of tuition is due with the exception of military transfer. If a parent is getting transferred, one month's written notification is required to be released from the contract. Signature*

By enrolling in the Virginia Beach School of the Arts, I am giving my consent for my child's likeness or photograph to be used in advertising and all promotional materials including newsletter and Facebook page. Signature*
I understand that payment of my Kindergarten supply fee of $150.00, will reserve their spot. Signature*
I understand that if my child is violent toward another child/teacher or repeatedly impedes the instructor's teaching through inappropriate behavior, tantrums, etc. they may be disenrolled without notice. Tuition is not refundable. Signature*
I understand that if my child is consistently disrespectful, consistently loud in class, impedes other student's learning, uses profanity or disrupts the peaceful atmosphere of the school, they will be removed from the classroom and possibly the program. Signature*
If my child has ADHD or any other suspected or diagnosed mental, academic delay or psychiatric condition, I will inform Virginia Beach School of the Arts staff immediately. Signature*
My child does not have an academic delay, ADHD, psychiatric condition, is not in need of special education or an IEP. Signature*
I acknowledge the Virginia Beach School of the Arts staff is not trained in Special Education or any other field involving meeting the needs of students with delays, autism, ADD/ADHD, ODD or similar. Signature*
I understand that Virginia Beach School of the Arts teachers and assistants are CPR and First Aid certified and have clear criminal background checks. The VBSA complies with requirements of student paperwork and facility safety standards by the State of Virginia and is not overseen by any government agency. Signature*
I understand that this is a homeschool enrichment program and that there will be course work sent home regularly. Signature*
I acknowledge my responsibility as a parent to check over homework, sign papers that come home for review, help my child study for all tests and will be actively involved with my child's education taking the ultimate, personal and final responsibility.
