COVID-19 Certificate of Vaccination:        Request Form

          This is the request form for COVID-19 Certificate of Vaccination of Hospital for Tropical Diseases only. Please prepare following document to complete this request form. ฟอร์มนี้ใช้สำหรับการยื่นขอ COVID-19 vaccine passport ที่โรงพยาบาลเวชศาสตร์เขตร้อนเท่านั้น) กรุณาเตรียมเอกสารดังต่อไปนี้ เพื่อใช้ในการกรอกข้อมูล

1. Passport valid for a minimum of 6 months (พาสปอร์ตที่มีอายุเกิน 6 เดือน)

2. Proof of COVID-19 vaccination in Thailand (ONLY!) (เอกสารยืนยันการรับวัคซีนโควิด19 ในประเทศไทย เท่านั้น!!

3. Document of the authorized representative who will pick up the certificate (if applicable) เอกสารยืนยันตัวตนของผู้รับมอบอำนาจในการรับหนังสือรับรองฯ ในกรณีที่ท่านจะให้ผู้อื่นมารับเอกสารแทน

4. Hospital registration number --> if you don't have one, please register at https://www.tropmedhospital.com/registration_en หมายเลขประจำตัวผู้ป่วยของโรงพยาบาล หากไม่มี กรุณาลงทะเบียนที่https://www.tropmedhospital.com/service/hospital-registration.html

1. We can certify ONLY COVID-19 vaccines receiving in Thailand, those administered outside Thailand will not be included on this certificate. (หนังสือรับรองฉบับนี้ สามารถรับรองได้เฉพาะวัคซีนโควิด 19 ที่ได้รับในประเทศไทยเท่านั้น ไม่สามารถรับรองวัคซีนที่ได้รับมาจากที่อื่น)

2. Failure to provide information requested on this form may result in significant processing delays and/or the denial of your application. (หากข้อมูล และเอกสารไม่ครบถ้วน อาจจะเป็นสาเหตุให้การออกหนังสือล่าช้ากว่ากำหนด และ/หรือ ไม่สามารถออกหนังสือรับรองฯได้)

3. This vaccine certificate is issued under the authority of Ministry of Public Health, Thailand. We can not gurantee that it would be recognized or accepted by other countries. หนังสือรับรองการฉีดวัคซีนฉบับนี้ออกภายใต้การรับรองจากกระทรวงสาธารณสุข ประเทศไทย ทางคลินิกไม่สามารถรับรองได้ว่าจะได้รับการยอมรับจากหน่วยงานของประเทศ

4. We will try every effort to let you get the certificate at your appointed date & time. However, there is no guarantee and we will not accept any responsibility for any delay, any loss or any other consequence either directly or indirectly related to the appointment. คลินิกจะพยายามให้ผู้ที่นัดหมายมาได้รับการบริการในเวลาที่กำหนด อย่างไรก็ตามอาจมีเหตุขัดข้องบางประการโดยไม่คาดคิด ดังนั้นท่านอาจจะต้องรอนานมากกว่าปกติ และทางคลินิกไม่รับผิดชอบผลที่ตามมาหากเกิดการล่าช้าขึ้น ไม่ว่าจะเป็นการเสียเวลา การพลาดนัดหมาย การพลาดการเดินทาง ฯลฯ
