  • Journey Transportation and Photo Release form

  • Transportation Release for Students: I hereby allow that the above named child/youth be permitted to attend Journey Christian Church sponsored events and ride as a passenger in transportation provided by Journey Christian Church. I also agree and consent to having the staff members, counselors, employees and adult sponsors, under whose auspices the program is conducted, and any other agent of Journey Christian Church in the program to secure any emergency medical care and/or treatment that may be necessary for my child during the entire youth event, including the trip to and from their destination. I further release that church staff, adult sponsors and organizers of the event from responsibility and liability for any accident or illness occurring during the event.
  • Photo Release for Students: Journey Christian Church has my permission to use the above-named child's photographs taken during any youth/missions event publicly to promote the ministry. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
  • Photo Release for Adults: Journey Christian Church has my permission to use my photographs taken during any youth event publicly to promote the ministry. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
