  • This form is only for new students to OUAZ and only needs to be completed once in your time with OUAZ.

    You will need to complete the Returner's Eligibility Form once a year (in the Fall) after your first academic year at OUAZ.

  • Please include the most accurate information possible. If you do not know the information or do not have the information requested write in "unknown."

  • Athletic Performance and Experience

  • NEW FOR THE 22-23 ACADEMIC YEAR: Beach Volleyball student-athletes will need to got through the PlayNAIA Eligibility Center and receive a determination of eligible.

    • If you plan to participate in cross country, indoor track and field, and outdoor track and field please select all three sports.
    • If you plan to participate in another sport/program not directly affiliated with your primary sport you will need to submit a Dual-Sport Participation request under the Requests & Forms tab of the OUAZCompliance.com website.
      • Examples include Indoor Women's Volleyball and Beach Volleyball, Weightlifting and Football, Water Polo and Swim....etc.

     

    • You do not need to submit a dual-sport participation request to compete between cheer, dance, and stunt.
  • You need to know what is expected of you and take responsibility for your eligibility. Your coach and the director of compliance are your support, but you are the one earning the credits and the GPA points.

  • High School Information

  • If you took the ACT or SAT please upload your test scores using the form on the "Transcripts and Test Scores" page of the OUAZCompliance website.

  • NAIA student-athletes are not allowed to compete against any non-OUAZ persons in any sport without being certified as eligible.

    Every student-athlete enters every new semester as NOT ELIGIBLE until proven eligible through the certification process.

    Do NOT compete against any non-OUAZ person in any sport until you know that you are allowed to do so.

  • NAIA Eligibility

  • Every student-athlete (except the sports listed below) must have an "eligible" determination from the NAIA and an "eligible" determination from OUAZ prior to competing.

    An NAIA determination is required only once while a student is in attendance at OUAZ.

    An OUAZ determination is required EVERY SEMESTER a student-athlete intends to compete.

     

    Sports that currently have no requirement for an NAIA determination:
    Weightlifting, Water Polo, Stunt, Cheer and Dance.

    If you earned hours at an institution while still in high school, list that school an write in YES for dual credits

  • Part of our checks and balances to avoid violations of any and all NAIA rules is to read the names of the eligible student-athletes from the completed certificate prior to the first competition.

    You will be eligible when you hear your name read out loud from a completed certificate by the director of compliance.

    If you don't hear your name read out loud by the DOC you are not-eligible and can suffer the consequences listed earlier.

  • In accordance with the Family Educational Rights and Privacy Act, I, the undersigned, hereby authorize the faculty athletics representative, athletics director and registrar of the institution I am attending to release any and all information about me which pertains to my eligibility to participate in intercollegiate athletics.

    The release of such information shall be restricted to any and all official representatives of the National Association of Intercollegiate Athletics (NAIA), and the conference and its member representatives.

    Such information is to be released only for the purpose of determining my eligibility for intercollegiate athletics, or for research or analytical purposes regarding student-athlete eligibility, academic progress or institutional administration of athletics.

    It is further understood that I may receive copies of such material from the institution upon request.
