    Gender/Name Update Request

    To ensure the accuracy and privacy of your record, the Department of Student Registration requires a written authorization to make changes or corrections to the name and/or gender listed in your student account.
    • Updating a name and/or gender with this request is not a legal name change.

    • The name that appears on the updated school records will be different from the legal name that may be required to apply for benefits and services outside the  FPS and this mismatch may create delays or issues.
