GENERAL MEMBERSHIP APPLICATION FOR DOG TRAINING

NST&ODC is a Positive, Rewards based dog training club 

Any sort of abusive behaviour is not allowed and we reserve the right to expel any member from the club who uses such behaviour towards dogs, children, other members, our volunteers or the public.

We also do not allow the use of check chains, prong collars or other aversive equipment during training so please bring along a normal lead, a flat collar and/or a properly fitted harness for your dog. 

THIS FORM IS FOR GENERAL REGISTRATIONS AND ADDITIONAL SPORT REGISTRATIONS.

^{Obedience training is held on Sunday mornings. Check our website for other training, class details and the calendar for start times, which change for summer and winter.}

^{Once your registration is submitted, you will receive a confirmation email. As there will not be any other communication please just come to training, at Carine Regional Open Space, on the first Sunday that you're able. Go to the desk near the trailer to collect your membership card and show your dog's vaccination ceritficate. The voluteers at the desk will let you know which class you'll be going to.}

Please Note:  You will need to bring the Vaccination Certificate, for each dog you are registering, along to the club the first time you come.

Any members who are returning from last year are still required to bring in an up-to-date vaccination certificate, for each dog, on the first Sunday they attend this year.

NST&ODC is committed to the protection of your personal information and follows the National Privacy principles for the fair handling of information. So that we can provide services to you, we ask for personal details such as name, address and telephone number. 

The database is used to provide an address list for the distribution of any correspondence required by the Club from time to time. In addition there is an "opt-in" to share your details with the Club sponsor, Better Pets and Gardens. We will not sell your name or contact details to a third party.

1.  I/We acknowledge that attendance at 

a) any dog training session
b) any trial or other function run by NST&ODC
C) the training grounds or any surrounding area

May involve risk of injury or damage to:

a) myself/yourself
b) others who may attend
c) my/our other dogs
d) and/or property

Even when all due care and consideration is taken and subject to clause 2 below, assume this risk.

2.  NST&ODC, in compliance with it's obligations under the Fair Trading Act, acknowledges and undertakes that it will exercise due care in carrying out the services provided.

3.  In consideration of NST&ODC agreeing to provide such services:
a) I/We exempt, in relation to the obligations of NST&ODC, it's volunteers, officers, members and agents ("the Club") from any liability whatsoever for any injury or damage resulting in any way, including as a result of negligence, and waive any rights I/we may have against the training organisation arising from such injury or damage.
b) Except in relation to the obligations of the Club, under the Fair Trading Act, I/we indemnify the Club against all claims which may arise from the Club oerforming any of the services it provides.

4.  I/We accept full responsibility for damages caused by myself when using any of the Club equipment with my/our dog/s.
