LATE PICK-UP FEES
Girl Summer Camp is open from 8:30 AM. - 6:00 PM. Any student, regardless of program participation, not picked up by end time of the session or 6:00 PM depending on agreement, will be charged $5 per student for the first minute late and then $1 per additional minute, per student. San Bernardino County Sheriff’s Department (SBCSD) will be notified if student(s) are not picked up within one hour of closing (7:00 PM) unless parent/guardian has appropriately communicated with Empowered to Flourish staff. After the 3rd late pick-up, at the discretion of the Program Coordinator, the student may be unenrolled from the program.
If at any time, Empowered to Flourish determines that the student is unable to benefit from the program, provides a safety risk for staff and/or students or impairs the ability of other students to benefit from the program, Empowered to Flourish may terminate this agreement. Notice will be given to the family and no refunds will be granted.
SIGN IN/OUT PROCEDURES
I understand that my student MUST be signed in and out each day (using the Empowered to Flourish attendance program) and may not be dropped off or picked up without signature. I also understand that my student may only be picked up by a parent/guardian or a other pre-authorized person indicated during registration.
No refund will be made for absences. Space is reserved for each participant. Staffing and daily activities are scheduled for the enrolled number of students. I understand that I will be charged for the sessions/weeks indicated in this agreement.
Breakfast/Lunch are not provided. Parents/guardians understand that it is their responsibility to provide a nutritious lunch for their student(s) on a daily basis. We discourage families from sending sodas and excessive sweets as students are very active during the day. Student lunches will not be refrigerated, please plan accordingly. Microwave ovens and other heating sources are not available.
Morning and afternoon snacks will be provided for all students in attendance.
If your student needs to take medication during program times it is important that you indicate this on the health question portion of Registration. All medication must be prescribed by a physician and appropriate forms and information shall be completed by parent/guardian. Camp staff will work with the student to administer the medication as prescribed by the physician. The Empowered to Flourish consent form for the administration of medicine must be completed by the physician and parent/guardian, and be on file at the Girl Summer Camp site. Medical Release and Authorization As Parent and/or Guardian of the named Child particpant, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to the Empowered to Flourish . and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility. Release authorized on the dates and/or duration of the registered season. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
ILLNESS AND EMERGENCY
Students that are ill or had a fever during the past 24 hours MAY NOT attend Girl Summer Camp. When my student becomes ill or injured, I will be notified and expected to pick them up promptly. In the event of an injury requiring emergency care, paramedics will be called and parents will be notified.
ELECTRONIC GAMES /ELECTRONIC DEVICES POLICY
Personal electronic devices and cellular devices including smart watches are not allowed at E2F Girl Summer Camp. For the safety of all students, electronic devices (including smart phones and watches) may not be used and will be held for return at the end of the day. In addition, any type of photography is prohibited by students unless it is a part of the enrichment being taught.
I understand that the Girl Summer Program is a fee-integrated program. Participation in the program is based upon the fulfillment of all requirements above regardless of program participation status. I also understand and agree that course availability is subject to change at the District's discretion. I understand that my student must attend and be on time to their enrolled sessions for the month of June.
Informed Consent and Acknowledgement I hereby give my approval for my child’s participation in any and all activities prepared by Empowered to Flourish during the selected camp. In exchange for the acceptance of said child’s candidacy by Empowered to Flourish., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Empowered to Flourish, Inc. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions. In case of injury to said child, I hereby waive all claims against Empowered to Flourish, Inc. including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
**Additionally, in the event that Empowered to Flourish must institute legal proceedings or other collection proceedings to enforce program fees owed, reasonable attorney and other fees paid in connection with the collection of program fees due on my account will be paid by the parent/guardian to the Empowered to Flourish.
Confirmation BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
