The Breathe Project: General Consent
PLEASE NOTE: This General Consent form gives permission for your child to attend any regular activities organised by The Breathe Project.

This includes Rock Steady, Rock Steady Plus, drop-in sessions, one-to-one sessions etc.

In the case of trips, holiday programmes and one-off sessions, specific consent forms will be required in order for your child to attend.
