There are no more appointments available for Jacksonville Dental Society's Give Kids a Smile 2023 at FSCJ.

Please consider signing your children up for dental hygiene care at the FSCJ Dental Hygiene Clinic on the college’s North Campus. Kids receive an exam, X-rays and a cleaning for $6! Sealants are also available to prevent decay for an additional fee. Adults receive the same care for $10! Call 904.766.6571 for additional information and to get on the waiting list for care.

The Jacksonville Dental Society's Give Kids a Smile event, hosted at Florida State College at Jacksonville's North Campus, will be held on Saturday, February 11.

1. Children MUST be screened for eligibility prior to being assigned an appointment for care on Saturday. Patient screening will be held on the date listed below. 

Screening Event 

Thursday, February 9, 5 - 8pm

The screening will consist of a review of the child's medical history, a brief oral exam, and a limited treatment plan for the Saturday event. Complete treatment is not possible on Saturday and follow-up care will likely be necessary at the child's dental office.

2. Parents or legal guardians must be present at the screening event and Give Kids a Smile Day for the child to receive care.

3. If a child is deemed eligible at the screening event, they will be appointed for a limited number of procedures (dental cleaning, fillings, extractions) on Saturday.

By completing this form, you are indicating that you would like to receive a screening appointment for your child/children. Appointments will be made on a first form submitted, first appointed basis.

4. You will be contacted by phone on Monday afternoon, February 6 if a screening appointment is available for your child/children. We must have verbal confirmation from the parent or legal guardian for the screening appointment. Once a screening appointment is made, an email confirmation will be sent detailing the appointment time and other instructions.

COVID Precaution

If any of the children or the parent/legal guardian have tested positive in the 10 days prior to the screening or are displaying any symptoms of COVID, please do not seek an appointment! A brief screening for COVID will take place for those who present to the campus for their screening.

1. Los niños deben ser examinados para pre-cualificar antes de obtener una cita de cuidado dental el día Sábado. 

Día para pre-cualificación:

Jueves, 9 de Febrero, 5-8pm

El examen consistirá en revisar la historia médica de los niños, hacer un examen corto de la boca, y un plan de tratamiento para el evento del sábado. En caso de que el tratamiento completo de la boca no se termine el sábado, se recomienda que se haga una cita de seguimiento con la oficina odontológica del niño.

2. El padre o representante legal del niño/a deberá estar presente durante el exámen de eligibilidad y también el día sábado durante el evento “Dale una Sonrisa a los Niños” para poder recibir el tratamiento correspondiente.

3. Si el niño/a es eligible el día de la pre-cualificación, se le dará una cita para el procedimiento determinado el cual puede incluir (limpieza dental, restauraciones, extracciones).

Completando esta forma, usted estará dando permiso para que su niño/a o niños reciban la cita para el examén de eligibilidad. Las citas serán dadas de acuerdo al orden de llegada.

4. Usted será contactado por teléfono el día lunes 6 de Febrero si hay una cita disponible para el exámen de eligibilidad para su niño/a. Nosotros debemos tener una confirmación verbal del padre/madre o representante legal del niño/a para hacer el exámen de eligibilidad.  Una vez hecha la cita, usted recibirá un correo electrónico con la hora de la cita y otras instrucciones.

Precauciones con el Covid

Si alguno de los niños, familiares o representantes legales de los niños ha salido positivo 10 días previo a la cita programada o ha tenido síntomas relacionados con el Covid, por favor NO SE PRESENTE a la cita!

Se le llamará a este teléfono en la tarde del día martes, 6 de Febrero para hacer la cita para el exámen de eligibilidad. Si no lo podemos contactar a este teléfono, no podremos hacer una cita para el exámen de eligibilidad de sus hijos.
