The following items must be submitted to qualify as a completed scholarship application submission. 

 

1. Application (Submitted Electronically)

2. Upload Photo

3. High School Transcript (Mailed/Hand Delivered)

4. Upload Essay

5. Upload Video Presentation

6. Upload Letter of Recommendation

7. Upload Acceptance Letter* from post-secondary institution (if not yet accepted,then please provide an official copy of submitted application confirmation from a college/university)

*If acceptance letter is not available at the time of application is submitted, it MUST be submitted prior to receiving funds.  Applicant will have until June 30th  to provide acceptance letter. 

8. Subsequent semester disbursement requirements acknowledgement

I hereby affirm that all the above stated information provided by me is true and correct to the best of my knowledge.   I also consent that my photo may be taken and used for any purpose deemed necessary to promote the Project Youth Educational (Y.E.S.), Inc. Scholarship Programs.

I hereby understand that (1) if chosen as a scholarship recipient and (2) to remain eligible, I must provide edividence outlined /articulated in the eligibility requirements and the application process connected to my enrollment/registration at the post-secondary institution of my choice before scholarship funds will be disbursed.
