By submitting this form, I acknowledge that the Board of Directors of Quince Orchard Park Community Association is relying on my representations herein in approving the use of the community facilities by the individual(s) named herein.

I additionally acknowledge and agree that any representations made herein or in connection with the assignment of my facility privileges shall subject me to all remedies available to the Board of Directors for violations of the rules and regulations of the Association, including but not limited to the assessment of charges and/or the suspension of my facility privileges, and such other remedies as may be established from time to time by the Board of Directors.

Each household will receive one keyfob per Authorized User free of charge. An administrative fee of up to $25.00 may be charged for each lost or stolen credential. 

Lost keyfobs must be reported immediately so that they may be disabled.  The homeowner will be liable for all individuals who access the facility with the keyfob issued for their property and any damage that may result.  Keyfobs should not be transferred to any person not listed below.

A new Agreement must be submitted whenever an Authorized User is added to or removed from the above list.  If this application is for a tenant, it must be submitted by the homeowner.  Tenant credentials must be renewed annually.
