COVID CONSENT
I understand that I must sanitize my hands before entering Lash Lounge and I must wear a SURGICAL mask that covers my mouth during the procedure, if I do not have one I will be purchasing one for $1
I confirm that I am not currently positive for novel coronavirus. I confirm that I am not waiting for the results of a laboratory test for the novel coronavirus.
I verify that I have not returned to California from any country outside of the US, whether by car, air, bus or train in the past 14 days.
I verify that I have not been identified as a contact of someone who has tested positive for the novel coronavirus or been asked to self-isolate by The Los Angeles Department of Health, or any other government agency.
I confirm that I am not presenting with any of the following symptoms of COVID-19 indentified by the CDC, (Fever > 100F, chills or body aches, Cough, Sore Throat Shortness of breath, Difficulty breathing, Flu-like symptoms, Runny Nose, Loss of smell or taste
RESCHEDULING POLICY
I understand that canceling my appointment 24 HOURS of my scheduled appointment results in a cancelation fee to LashLashLounge.Lrose of $40 that I must pay
I understand that canceling my appointment 48 HOURS of my scheduled appointment results in a cancelation fee to LashLashLounge.Lrose of $20 that I must pay
I understand that my deposit of $15 is non refundable and non transferable, if I choose a different date after 24HOURS of booking, I must pay another deposit fee of $15
CONSENT
I understand that this procedure requires eyelash extensions to be glued onto my own natural eyelashes and that there is a possibility of an allergic reaction; if I do experience one I will contact @LashLounge.lrose within 48 hours to assess the situation, but will not receive a refund for my service but can get a free removal at her soonest availability.
I understand that there are some risks to this procedure and it may be, but not limited to eye redness and irritation. The fumes from the adhesive may also cause my eyes to water.
I understand that my appointment may take up to 3 hours depending on the services requested.
I understand that @Lashlounge.lrose is not licensed, but is multi certified eyelash extension technician and I allow her to perform the eyelash procedure on me at my own risks.
Payment / Booking Policy
I understand that before my appointment can be approved I need to send a $15 nonrefundable deposit
I understand that my remaining balance due the day of my appointment is CASH ONLY
I understand that I have booked accordingly on Lash Lounge booking, filled out my consent form and sent my deposit.
I have fully reviewed Lash Lounge Policys on her story instagram highlights and I agree and respect her terms.
