This next section is for proof of vaccine. As you may know, there have been a lot of outbreaks this year. Since everyone typically does a 5 way or 6 way at this time of year this should not be a problem to provide proof of vaccine specifically for EHV & EHV 1-4 (flu rhino).

PROOF OF VACCINE IS REQUIRED AND MUST BE SUBMITTED WITH ENTRY AS REQUESTED BY FACILITY.

Individual riders can upload the proof as per outline below by vet certificate or documentation and signature.

If your horse is vaccinated as part of herd health and you are leasing or riding a school horse you may not have access to that information.

IF your coach will be providing one document to cover all entries from your facility please let us know.
Person Responsible Signature: I hereby certify that every horse listed on this entry form has met therequirements of Article A519, Vaccinations. All horses attending the competition must have been administered Equine Influenza and Equine Herpes Virus (1 and 4) vaccinations within six (6) months (+21 days grace period)before arrival at the competition. No horse shall have received vaccination within seven (7)days prior to arrival at the event. Horses not in compliance with this rule may be asked to leave event site at the discretion of competition management. If administered by a veterinarian, the veterinarian will provide documentation to the Person Responsible. If the veterinarian did not administer the vaccine, the Person Responsible will provide: the horse's name, name of the vaccine with serial number and expiration date, the date administered, and signature confirming that all information is true and correct. This can be done with a simple photo and uploaded to this entry. and signature below.*
