    The information below is to help protect your children or teens from concussion or other serious brain injury. Use this information at your children’s or teens’ games and practices to learn how to spot a concussion and what to do if a concussion occurs.

    WHAT IS A CONCUSSION?

    A concussion is a type of traumatic brain injury—or TBI—caused by a bump, blow, or jolt to the head or by a hit to the body that causes the head and brain to move quickly back and forth. This fast movement can cause the brain to bounce around or twist in the skull, creating chemical changes in the brain and sometimes stretching and damaging the brain cells.

    HOW CAN I HELP KEEP MY CHILDREN OR TEENS SAFE?

    Sports are a great way for children and teens to stay healthy and can help them do well in school. To help lower your children’s or teens’ chances of getting a concussion or other serious brain injury, you should:

    *Help create a culture of safety for the team.Work with their coach to teach ways to lower the chances of getting a concussion.
    *Talk with your children or teens about concussion and ask if they have concerns about reporting a concussion. Talk with them about their concerns; emphasize the importance of reporting concussions and taking time to recover from one.
    *Ensure that they follow their coach’s rules for safety and the rules of the sport.
    *Tell your children or teens that you expect them to practice good sportsmanship at all times.
    *When appropriate for the sport or activity, teach your children or teens that they must wear a helmet to lower the chances of the most serious types of brain or head injury. However, there is no “concussion-proof” helmet. So, even with a helmet, it is important for children and teens to avoid hits to the head.

    HOW CAN I SPOT A POSSIBLE CONCUSSION?

    Children and teens who show or report one or more of the signs and symptoms listed below—or simply say they just “don’t feel right” after a bump, blow, or jolt to the head or body—may have a concussion or other serious brain injury.

    Signs Observed by Parents or Coaches

    Appears dazed or stunned.
    Forgets an instruction, is confused about an assignment or position, or is unsure of the game, score, or opponent.
    Moves clumsily.
    Answers questions slowly.
    Loses consciousness (even briefly).
    Shows mood, behavior, or personality changes.
    Can’t recall events prior to or after a hit or fall.
    Symptoms Reported by Children and Teens

    Headache or “pressure” in head.
    Nausea or vomiting.
    Balance problems or dizziness, or double or blurry vision. 
    Bothered by light or noise.
    Feeling sluggish, hazy, foggy, or groggy.
    Confusion, or concentration or memory problems.
    Just not “feeling right,” or “feeling down.”

    Talk with your children and teens about concussion. Tell them to report their concussion symptoms to you and their coach right away. Some children and teens think concussions aren’t serious or worry that if they report a concussion they will lose their position on the team or look weak. Be sure to remind them that it’s better to miss one game than the whole season.

    Concussions a ect each child and teen differently. While most children and teens with a concussion feel better within a couple of weeks, some will have symptoms for months or longer. Talk with your children’s or teens’ health care provider if their concussion symptoms do not go away or if they get worse after they return to their regular activities.

    WHAT ARE SOME MORE SERIOUS DANGER SIGNS TO LOOK OUT FOR?

    In rare cases, a dangerous collection of blood (hematoma) may form on the brain after a bump, blow, or jolt to the head or body and can squeeze the brain against the skull. Call 9-1-1 or take your child or teen to the emergency department right away if, after a bump, blow, or jolt to the head or body, he or she has one or more of these danger signs:

    *One pupil larger than the other.
    *Drowsiness or inability to wake up.
    *A headache that gets worse and does not go away.
    *Slurred speech, weakness, numbness, or decreased coordination.
    *Repeated vomiting or nausea, convulsions or seizures (shaking or twitching).
    *Unusual behavior, increased confusion, restlessness, or agitation.
    *Loss of consciousness (passed out/knocked out). Even a brief loss of consciousness should be taken seriously.

    Children and teens who continue to play while having concussion symptoms or who return to play too soon—while the brain is still healing— have a greater chance of getting another concussion. A repeat concussion that occurs while the brain is still healing from the rst injury can be very serious and can a ect a child or teen for a lifetime. It can even be fatal.

    What Should I Do If My Child or Teen Has a Possible Concussion? As a parent, if you think your child or teen may have a concussion, you should:

    1. Remove your child or teen from play.
    2. Keep your child or teen out of play the day of the injury. Your child or teen should be seen by a health care provider and only return to play with permission from a health care provider who is experienced in evaluating for concussion.
    3. Ask your child’s or teen’s health care provider for written instructions on helping your child or teen return to school. You can give the instructions to your child’s or teen’s school nurse and teacher(s) and return-to-play instructions to the coach and/or athletic trainer.

    Do not try to judge the severity of the injury yourself. Only a health care provider should assess a child or teen for a possible concussion. Concussion signs and symptoms often show up soon after the injury. But you may not know how serious the concussion is at rst, and some symptoms may not show up for hours or days. The brain needs time to heal after a concussion. A child’s or teen’s return to school and sports should be a gradual process that is carefully managed and monitored by a health care provider.

  • YOUTH SPORTS CONCUSSION AND
    SUDDEN CARDIAC ARREST PREVENTION PROTOCOLS

    On August 30, 2019 an act to amend Section 124235 of, and to amend the heading of Article 2.5 of Chapter 4 of Part 2 of Division 106 of, the Health and Safety Code, relating to youth athletics was approved by the governor of California.

    Article  2.5. Youth Sports Concussion and Sudden Cardiac Arrest Prevention Protocols

    SEC. 2.  Section 124235 of the Health and Safety Code is amended to read:

    124235. (a) A youth sports organization that elects to offer an athletic program shall comply with all of the following:
    (1)(A) An athlete who is suspected of sustaining a concussion or other head injury, or who has passed out or fainted, in an athletic activity shall be immediately removed from the athletic activity for the remainder of the day, and shall not be permitted to return to any athletic activity until the athlete is evaluated by a licensed healthcare provider. The athlete shall not be permitted to return to athletic activity until the athlete receives written clearance to return to athletic activity from a licensed healthcare provider. If the licensed healthcare provider determines that the athlete sustained a concussion or other head injury, the athlete shall also complete a graduated return-to-play protocol of no less than seven days in duration under the supervision of a licensed healthcare provider.
    (B) If the licensed healthcare provider suspects that the athlete has a cardiac condition that puts the athlete at risk for sudden cardiac arrest or other heart-related issues, the athlete shall remain under the care of the licensed healthcare provider to pursue followup testing until the athlete is cleared to play.
    (2) If an athlete who is 17 years of age or younger has been removed from athletic activity due to a suspected concussion or due to fainting or another suspected cardiac condition, the youth sports organization shall notify a parent or guardian of that athlete of the time and date of the injury, the symptoms observed, and any treatment provided to that athlete for the injury.
    (3) (A) On a yearly basis, the youth sports organization shall give both a concussion and head injury and a sudden cardiac arrest information sheet to each athlete. The information sheet shall be signed and returned by the athlete and, if the athlete is 17 years of age or younger, shall also be signed by the athlete’s parent or guardian, before the athlete initiates practice or competition.
    (B) If the athlete is six years of age or younger, only the signature of the athlete’s parent or guardian shall be required to comply with this paragraph. If the athlete is 18 years of age or older, only the signature of the athlete shall be required to comply with this paragraph.
    (C) The information sheet may be sent and returned through an electronic medium including, but not necessarily limited to, fax or electronic mail.
    (4) On a yearly basis, the youth sports organization shall offer concussion and head injury and sudden cardiac arrest prevention education, or related educational materials, or both, to each coach, administrator, and referee, umpire, or other game official of the youth sports organization.
    (5) The youth sports organization shall require both of the following:
    (A) Each coach, administrator, and referee, umpire, or other game official of the youth sports organization shall be required to successfully complete the concussion and head injury and sudden cardiac arrest prevention education offered pursuant to paragraph (4) at least once, either online or in person, before supervising an athlete in an activity of the youth sports organization.
    (B) The youth sports organization shall post related information, as referenced in paragraph (4), online, or provide educational materials to athletes and parents, or both.
    (6) The youth sports organization shall identify both of the following:
    (A) Procedures to ensure compliance with the requirements for providing concussion and head injury and sudden cardiac arrest prevention education and a concussion and head injury and sudden cardiac arrest prevention information sheet, as referenced in paragraphs (3) to (5), inclusive.
    (B) Procedures to ensure compliance with the athlete removal provisions and the return-to-play protocol required pursuant to paragraph (1).
    (b) As used in this article, all of the following shall apply:
    (1) “Concussion and head injury education and educational materials” and a “concussion and head injury information sheet” shall, at a minimum, include information relating to all of the following:
    (A) Head injuries and their potential consequences.
    (B) The signs and symptoms of a concussion.
    (C) Best practices for removal of an athlete from an athletic activity after a suspected concussion.
    (D) Steps for returning an athlete to school and athletic activity after a concussion or head injury.
    (2) “Licensed healthcare provider” means either of the following:
    (A) A licensed healthcare provider who is trained in the evaluation and management of concussions and is acting within the scope of the provider’s practice for evaluation and management of concussions or other head injuries.
    (B) A licensed healthcare provider who is trained in the evaluation and management of cardiac conditions and is acting within the scope of that provider’s practice for evaluation and management of sudden cardiac arrest, fainting, and shortness of breath.
    (3) “Sudden cardiac arrest prevention education and educational materials” and a “sudden cardiac arrest information sheet” shall, at a minimum, include information relating to all of the following:
    (A) Cardiac conditions and their potential consequences.
    (B) The signs and symptoms of sudden cardiac arrest.
    (C) Best practices for removal of an athlete from an athletic activity after fainting or a suspected cardiac condition is observed.

    (D) Steps for returning an athlete to an athletic activity after the athlete faints or experiences a cardiac condition.
    (E) What to do in the event of a cardiac emergency: this shall include calling 911, performing hands-only CPR, and using an automated external defibrillator (AED) if it is available.
    (4) “Youth sports organization” means an organization, business, nonprofit entity, or a local governmental agency that sponsors or conducts amateur sports competitions, training, camps, or clubs in which persons 17 years of age or younger participate.
    (c) This section shall apply to all persons participating in the activities of a youth sports organization, irrespective of their ages. This section shall not be construed to prohibit a youth sports organization, or any other appropriate entity, from adopting and enforcing rules intended to provide a higher standard of safety for athletes than the standard established under this section.

    Fountain Valley PONY Baseball and PONY Baseball are now requiring all team managers and team assistant coaches to be concussion trained and sudden cardiac arrest trained. On an annual basis and prior to the start of the season, all team managers and team assistant coaches will need to be trained and/or retrained and will need to provide a certificate of course completion to Fountain Valley PONY Baseball to keep on file.

  • WAIVER & LIABILITY RELEASE MANAGERS & COACHES

    NOTE: THIS FORM MUST BE READ AND SIGNED BEFORE THE PARTICIPANT IS PERMITTED TO TAKE PART IN ANY FVPB EVENTS, PRACTICES, OR GAMES. BY SIGNING THIS AGREEMENT, THE PARTICIPANT AFFIRMS HAVING READ IT.

    IN CONSIDERATION of being allowed to participate in any way in FOUNTAIN VALLEY PONY BASEBALL under the auspices of Pony National ,I, the undersigned, acknowledge, understand, and agree that:

    1. I RISK BODILY INJURY, INCLUDING PARALYSIS, DISMEMBERMENT, DISABILITY AND DEATH, AND while particular rules of the sport, skills, equipment, and personal discipline may reduce this risk, THIS RISK OF SERIOUS INJURY DOES EXIST, AS WELL AS THE RISK OF DAMAGE TO OR LOSS OF PROPERTY; and,

    2. I KNOWINGLY AND FREELY ASSUME ANY AND ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERS, and assume full responsibility for my participation; and,

    3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, or if I observe any concern in my readiness for participation, I will immediately bring such to the attention of the nearest official and remove myself from participation; and,

    4. I, FOR MYSELF, and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE, INDEMNIFY, HOLD HARMLESS and PROMISE NOT TO SUE FOUNTAIN VALLEY PONY BASEBALL OR PONY NATIONAL, their officers, officials, volunteers, employees, agents, and/or other participants, sponsors, advertisers, and, if applicable, the owners and lessors of premises used for the activity ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, and/or LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, EXCEPT THAT WHICH IS THE RESULT OF GROSS NEGLIGENCE OR WANTON MISCOUNDUCT, to the fullest extent permitted by law.

    I have read this Release of Liability and Waiver Agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.

  • MANAGER’S & COACH’S CODE OF CONDUCT

    All Managers and Coaches, as a condition to their selection and continued service as Managers & Coach’s, are expected to adhere to the following Codes of Conduct.

    1. Managers are to recognize that they are role models and examples for the players and conduct themselves accordingly.
     Show good sportsmanship at all times.
     No swearing or abusive language.
     No ridicule or harassment of umpires.
     No verbal abuse or ridicule of any player.
     No physical abuse (pushing, grabbing or shoving) of any player, coach or umpire.
     
    2. There will be no changing or adjusting to their team’s roster, any and all changes will be made by the league’s Player Agent.
    3. The manager is responsible for all the players on his/her team.  No ridicule of each other or of those players on the opposite team will be tolerated.  Constructive criticism is the responsibility of the manager, not the players.
    4. The manager is responsible for his/her teams assistant coach, teams parents, and teams spectators while team is on the field.
    5. The player’s conduct on the field and the dugout is the responsibility of the manager.
    6. The manager should encourage participation by all the players, especially and including the weaker players.  Never discourage a player’s involvement.  Each player should feel they are an important of the team.
    7. Please direct all issues of conflict to the division Commissioner, who will be responsible for the resolution of all division issues.
    8. If a manager and/or coach is found to be slandering FVPB in a manner that FVPB feels is not in the best interest of the league, that manager/coach may be asked to leave the league.

    Manager’s and/or Coach’s can and will be relieved from participation in the FVPB program if it is determined that their conduct violated either the letter or the spirit of the Code of Conduct or the Rules and Regulations to which this code is part.


  • PARENTAL AUTHORIZATION MEDICAL RELEASE FOR
    PARTICIPATION IN PONY BASEBALL ACTIVITIES

    I, as the parent or guardian of the player do hereby give my approval for their participation in any and all PONY BASEBALL league activities. I hereby grant my permission to managing personnel or other league representatives to authorize and obtain medical care, at my expense, from any licensed physician, hospital or medical clinic should the player become ill or injured while participating in league activities away from home, or where neither parent or legal guardian is available to grant authorization for emergency treatment.

    I assume all risks and hazards incidental to my child’s participation, including transportation to and from the activities; and do hereby waive, release, absolve, indemnify and agree to hold harmless the local PONY BASEBALL, INC organization, PONY BASEBALL, INC, the organizers, sponsors, supervisors, participants and persons transporting the player to and from the activities, for any and all claims arising out of an injury to the player.

    I further agree to furnish a certified birth certificate for the player, upon request of league officials, and to return upon request the uniform and other equipment issued to the player in as good a condition as when received, except for normal wear and tear in league activities.

  • WAIVER & LIABILITY RELEASE PLAYERS

    I/We, the parent/guardian of the named player being registered, know that participation in baseball may result in serious injuries and protective equipment does not prevent all injuries to players. I hereby waive, release, absolve, indemnify, and agree to hold harmless PONY Baseball, Inc., Fountain Valley PONY Baseball, its directors, coaches, organizers, sponsors, and participants for any claim arising out of any injury to my child, whether the result of negligence or for any other cause, except to the extend and in the amount covered by accident or liability insurance. Further, I hereby grant permission to the adult manager, coach, trainer or business manager of the team to obtain medical care, at my expense, from any licensed physician, hospital or medical clinic, for the player named herein at such time as I cannot be contacted in person or by telephone. This authorization shall include all league activities, including travel to and from all league activities.

    I the parent/guardian of the named player being registered also understand Fountain Valley PONY Baseball’s Refund Policy: All refunds will be assessed a $30 processing fee. Registration fees will not be refunded after the final sign-up date unless there is a valid medical reason or a player is moving outside of the area. If a uniform has already been issued to a payer, the player shall keep the uniform and $50 will be deducted from any fee refund. Registration fees will not be refunded after opening day of the season in question under any circumstances. Any credit card chargebacks will result in the named player being registered being removed from the assigned team until registration payment has been received with cash only.

    I, the undersigned, hereby give permission to Fountain Valley PONY Baseball, and/or parties designated by Fountain Valley PONY Baseball to photograph and videotape my child/children and to use such photographs and videotapes in all forms of media, for any and all purposes including advertising, audiovisual exhibition, television commercials, community service announcements, press advertisements, print publications, public relations displays, trading manuals, strategic plans, posters, and promotional displays in public venues. I further consent to the use of my child’s/children’s name in connection with the photographs or videotape on the league website. I understand that there will be no financial compensation for the use of the photographs, videotapes, and/or the name of my child/children. I understand that this consent shall remain valid until I withdraw my consent in writing and I hereby release Fountain Valley Pony Baseball from any and all claims that may arise out of the use of the photographs, videotape and/or name of the person named below. I also grant Fountain Valley Pony Baseball the right to exclude 3rd party photographers from photographing or videotaping my child/children on any premise owned, leased or rented by Fountain Valley Pony Baseball.

  • FVPB PARENT CODE OF CONDUCT

    We, Fountain Valley Pony Baseball, have implemented the following Parent Code of Conduct for the important message it holds about the proper role of parents in supporting their child in sports. Parents shall read, understand and sign this form prior to their children participating in our league.

     Any parent guilty of improper conduct at any game or practice will be asked to leave the sports facility and may be suspended from the following game. Repeat violations may cause a multiple game suspension, or the season forfeiture of the privilege of attending all games.

    Preamble
    The essential elements of character building and ethics in sports are embodied in the concept of sportsmanship and six core principles: 
      Trustworthiness, Respect, Responsibility, Fairness, Caring, and Good Citizenship.
      The highest potential of sports is achieved when competition reflects these “six pillars of character.”

    I therefore agree:

    1. I will not force my child to participate in sports.
    2. I will remember that children participate to have fun and that the game is for youth, not adults.
    3. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.
    4. I will learn the rules of the game and the policies of the league.
    5. I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game and practice.
    6. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures.
    7. I will not encourage any behaviors or practices that would endanger the health and well being of the athletes.
    8. I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence.
    9. I will demand that my child treat other players, coaches, officials and spectators with respect regardless of race, creed, color, sex or ability.
    10. I will teach my child that doing one’s best is more important than winning, so that my child will never feel defeated by the outcome of a game or his/her performance.
    11. I will praise my child for competing fairly and trying hard, and make my child feel like a winner every time.
    12. I will never ridicule or yell at my child or other participants for making a mistake or losing a competition.
    13. I will emphasize skill development and practices and how they benefit my child over winning. I will also de-emphasize games and competition in the lower age groups.
    14. I will promote the emotional and physical well being of the athletes ahead of any personal desire I may have for my child to win.
    15. I will respect the officials and their authority during games and will never question, discuss, or confront the officials.
    16. I will provide only supportive comments to coaches and avoid any derogatory comments. If I have a concern(s) to bring to the coach's attention, I will apply the "24 Hour Rule" before having a respectful and private conversation at an agreed upon time and place.
    17. I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all sports events.
    18. I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team.
