    you have filled out this form accurately and correctly
    you are the original owner of the item and have the original copy of the proof of purchase (sales invoice/email order confirmation)
    you have read, understood and complied with the warrranty coverage, maintenance, assembly and owner's manual of your purchased item
    you will provide any and all additional information that Raptor may require to file your warranty claim, to diagnose your issue and to fulfill the warranty claim
    you understand and bear all responsilibilty for any incorrect or incomplete information in this form and further submissions that may result to a rejection of your warranty claim request and that, if you still want to file the same claim after rejection, you will need to submit a new request form with corrrected and completed details.
