    By signing below and submitting this after sales request form, you are agreeing that:

    1. you have purchased the item from Raptor or its authorized dealers. And that you understand that Raptor reserves the right to refuse to provide you with after sales parts and service for items not purchased from Raptor and its authorized dealers.

    2. you have filled out this form accurately and correctly. And that you understand and agree to bear all responsilibilty for any incorrect or incomplete information in this form and further submissions that may result to an incorrect quotation, parts/service incompatibility or request rejection.

    3. you will provide any and all additional information that Raptor may require to identify your product and the part/ service request and to diagnose your issue.
