Photo Release Form for Minors

I give Very Important Princess Mobile Spa Experience LLC., permission to use my child’s likeness, image, voice, and/ or appearances as such may be embodied in any pictures, photos, video recordings, digital images, and the like, taken or made on behalf of Very Important Princess Mobile Spa Experience LLC., party activities. I agree that Very Important Princess Mobile Spa Experience LLC., has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Very Important Princess Mobile Spa Experience LLC. business. These uses include, but are not limited to, reprints, publications, advertisements, social media and any promotional materials in any medium now known or later developed, including the internet. I acknowledge that I will not receive any compensation for the use of such pictures, etc., and hereby release Very Important Princess Mobile Spa Experience LLC., from any and all claims which are in any way connected with such use. I have read and understood this consent and release.

 
Makeup Consent

 
As part of the Very Important Princess Mobile Spa Experience LLC. party experience age appropriate makeup and spa treatment products will be applied. VIP Parties are for entertainment purposes only. We exercise special care in being sanitary and using mild kid-friendly products that are especially sensitive to young skin. Although, we make no claims of being licensed professional technicians, cosmetologists or estheticians. Therefore, we take no responsibility for any allergic reactions of any party guest. It is the responsibility of the parent or guardian hosting the party to alert VIP in advance of any allergies that any of the children may have. VIP reserves the right to refuse services on any child, for any reason, that we feel may impose a health risk to others. Our products are handmade using only the finest quality ingredients from start to finish, every product is made with natural, mild ingredients, and are: 95% or More natural, Gluten Free, Paraben Free & Cruelty Free! I have read and understood this consent and release.

 

Food Allergy Permission

 
Your child will be participating in various activities on the VIP Spa Bus. Additionally, some of the activities involve food products as part of the experience. VIP takes every precaution to ensure that our girls are safe. Very Important Princess Mobile Spa Experience LLC, will not be responsible for any allergic reactions or life threatening incident including death that could occur. If your child has any known food allergies that may prevent her from participating in these activities, we will provide an alternative option per your request. I have read and understood this consent and release.

  

By checking the box and booking a party with Very Important Princess Mobile Spa Experience LLC it confirms I have read and understood this consent and release and assume all responsibility as set herein.

I, the undersigned participant, affirm that I am of the age of 18 years or older, and that I am freely signing this agreement. I certify that I have read this agreement, that I fully understand its content and that this release cannot be modified orally. I am aware that this is a release of liability and a contract and that I am signing it of my own free will.
