SCHOLARSHIP INFORMATION:

Recipients:

- Two Black graduating Seniors in good standing of Forest Park High School

Award Amount:

- $1,500 per recipient

Eligibility:

- Must be a graduating senior in good standing at Forest Park Senior High School

- Cumulative GPA: Minimum 3.0 preferred (unofficial transcript required)

- Must plan to pursue a degree from an accredited 2-year/4-year College or University

- Resume/CV

- Two letters of recommendation, one must be from a teacher (no family or peers)

- 2-3 minute Video Submission

In 2-3 minutes max, reflect and expand upon the following items:

- What makes you unique?

- What extracurricular activities are you involved in?

- How will you use your education to further impact the Black community?

- Who is your favorie Black role model and why?
