^{The undersigned agrees: To provide adequate food, water, shelter, safe containment, and humane treatment of the animal(s) at all times. To monitor the animal(s) and provide proper care and socialization to enhance the lives of both the animal(s) and future fosters/adopters. To notify LAP staff immediately and bring the animal(s) back to LAP for treatment if they become ill. The foster understands that if the foster takes the animals to a private veterinarian without prior approval, they assume the cost of treatment and will not be reimbursed in any way by LAP. Copies of treatment by the private veterinarian must be provided to LAP.}

^{To bring the animal(s) back to LAP at the end of the foster period or adopt the animal(s) as specified. Extensions of the foster period may only be made by LAP staff on a case by case basis. To bring the animal(s) to Wellness Clinic and Veterinary appointments for vaccinations, microchipping, spay/neuter, and other necessary care as needed. That fostered animals are property of LAMESAADOPTABLE PETS. This agreement transfers no ownership rights. Ownership can only be transferred through a completed adoption application with payment in full. That if a fostered animal dies, the body must be returned to LAP or the foster caregiver must provide verification of death by a veterinarian or other legal authority. The undersigned further agrees:}

^{To keep personal and fostered animals up to date on the following vaccinations:}

^{o Canines: Distemper, Parvovirus, Parainfluenza, Hepatitis, Heartworm, Rabies}

^{o Felines: Feline Distemper, Pneumonitis, Rhinotracheitis, Calici, Feline Leukemia, Rabies}

^{To insure personal and fostered animals are kept free of all parasites.}

^{I understand LAP will not reimburse for any expenditures I incur for the care and treatment of my own animal(s). I understand that if a fostered animal contracts a contagious disease, the foster home will not be considered for further fostering other animals for a length of time to be determined by LAP.}

^{I also understand that if I brought the animal I am fostering into LAP ( as my own animal that I owner surrender and I agree to foster) if at any time I cannot/will not continue to foster said animal, I will pay a portion of their medical care to LAP so that my responsibilities for this animal cease and another foster home or boarding can be found.}

^{I agree to release, discharge, indemnify, and hold harmless LAMESA ADOPTABLE PETS, including its agents, employees and volunteers for any and all personal injuries, damages to property or pets caused by the foster animals. I recognize that in handling foster animal(s) there exists risk of personal injury, including physical harm caused by the foster animal(s). On behalf of myself, my heirs, personal representatives, and executors, I release, discharge, indemnify, and hold harmless LAP, its agents, volunteers and employees from any and all claims, causes of actions, or demands, or any natures of cause connected with my foster care agreement.}

^{LAMESAADOPTABLE PETS (LAP) makes no guarantee or representation about the selected animal's temperament and cannot be held responsible (liable) for any injuries, bites, or damages caused by the selected animal while under the control of the undersigned. LAP may remove fostered animals from foster care for failure to comply with any portion of this agreement or if LAP representatives feel it is in the best interest of the animals.}

By signing below you verify that all the information you provided is accurate to the best of your knowledge. You also agree to abide by LAMESA ADOPTABLE PETS Care Agreement and Indemnity and to periodic home visits to inspect the welfare and environment of the animals you are fostering. You also understand that the animal you are fostering belongs to LAMESA ADOPTABLE PETS until the adoption paperwork is completed and payment is made in full. The fostered animal may be removed as deemed necessary by LAMESA ADOPTABLE PETS staff.

Submission of this application is NOT a guarantee of foster, and all placements will be chosen on a case by case basis, not first come first serve. We reserve the right to place the pet with the home that fits the pet best.
