This form is used to request that a tenant be given notice to vacate so that a new tenant can be placed. This form is NOT used to terminate management. If termination of the management contract is desired, please use the form for management termination.

NOTE: We need at least two (2) full calendar months' notice to terminate a lease. If we are already past that deadline, we will not be able to non-renew.
