FINANCIAL OBLIGATIONS & PAYMENT SCHEDULE
$200 due on Sunday, January 30, 2022 (includes registration, 1st month and rehearsal attire)

$150 due on or befor Tuesday, March 1, 2022

$150 due on or before Friday, April 1, 2022

$150 due on or before Sunday, May 1, 2022

$150 due on or before Wednesday, June 1, 2022

***After the 5th of each month, payments will be considered late and a $25 late fee will be assessed. Dancers will not be able to participate until fees are paid.

CONCERT FEES

$200 due on or before Sunday, May 15, 2022

TICKET MONEY

Due on or before Saturday, June 4, 2022

T-SHIRT MONEY

$12 due on or before June 4, 2022

All dancers are required to sell 6-10 tickets. Tickets are $30 each. Exact amount will be provided at a later time.

I blanks* understand and accept the financial requirements set forth by the Young Contemporary Dance Theatre, LLC. I also understand that any dancer that fails to satisfy all financial commitments may result in the exclusion of the company's final performance, tentative concert date TBA. By signing this financial obligation form, I agree to submit payments in the full amount on the dates provided above. I understand and agree that all payments are non-refundable (personal checks will not be accepted). I understand that masks are required in the Supa Studio and a COVID-19 form will be provided on the day of the audition. ALL DANCERS MUST BE PICKED UP ON TIME. YOUNG CONTEMPORARY DANCE THEATRE WILL NOT BE RESPONSIBLE FOR DANCERS LEFT AFTER REHEARSAL IS OVER.
