Initial I certify that the above information is true and correct. I understand that any falsification of the above information may be grounds for denial of this application or termination. I acknowledge that this application will remain the property of AAAR&T.

Initial I agree to vaccinate, feed, love, and keep my pet on all preventative medications (flea/tick/heartworm), exercise my pet as needed and to keep identification tags on him/her. I also agree to have my pet checked out by my personal veterinarian within 30 days of adoption and provide proof to AAAR&T or one of their members/volunteers via email or text.

Initial I agree never to allow this animal to be used for the purposes of the following: vivisection, breeding, experimentation, dog biting or fighting, as strictly outdoor guard dog, or any other purpose except as a house pet and companion.

Initial I understand that previous temperament is not always an indication of future temperament. I agree to release, hold harmless and forever discharge AAAR&T and its agents and employees of any and all liability, property damage, medical costs, and all known or unknown, anticipated or unanticipated, suspected or unsuspected causes of action, charges, suits, debts, demands, claims, liabilities, losses, costs, expenses (including, without limitation, attorneys' fees) or damages, including but not limited to any medical costs, date to property, persons or other pets, of any and every kind, nature and description, at law or in equity, in connection with or arising from while I am caring for the agreed rescue companion animal. I take full responsibility for the care of this pet and any damage or injury he/she may cause or illness that he/she may encounter.

Initial Adoption fee starts at $400.00 and can be higher due to vetting, etc.
This fee includes all age appropriate vaccinations, spay/neuter voucher and microchip on dogs younger than 6 mths.
A spay/neuter (if older than 6 months of age); as well as the first month of flea and heartworm preventative. Full amount of the fee is due at the time of adoption.

Initial Proof of spay or neuter completed by adopter is to be sent to AAAR&T as soon as it's completed. Failure to do this may result in surrendering the dog back to the care of the AAAR&T organization.

Initial Upon completion of the adoption, the adopter will notify the company who provided the microchip and complete the registration for the dog to be listed at the correct home address and owner information. This is to be completed by the adopter within 14 days of adoption.

Initial OUT OF STATE ADOPTIONS ARE AT THE ADOPTERS' EXPENSE. I Understand that as an out of state adopter I am responsible for transport, health cert. , etc. Out of state adoptions must agree to live home checks and be present for a meet n greet.
