I hereby acknowledge that I am participating in a training class for dogs. I further acknowledge that such a class involves an element of risk in both the on leash and off leash exercises. Therefore, I absolve the instructor, assistants, and owners of the premises as well as other participants of responsibility or liability for any accidents of injuries that may result from said activities..*

I acknowledge the above and agree to continue with the classmy dog's vaccinations are up to date and in accordance with Ontario laws. For puppies under 4 months I have at least 1 set of immunizations completed.I confirm that my dog has NO history of biting or aggression towards other dogs or people

I agree to pay the full amount for my program via email transfer to carolthedogwalker@gmail.com prior to my first session. Note that auto-deposit is enabled and the account name is "Mary Carol Upton" so don't panic.I don't do email transfers so I will make other arrangements for the payment for my program.
